colchicine (KOLE-chih-sene)
Type of Drug Antigout medication.
Prescribed For
Prevention and treatment of gouty arthritis; also prescribed for Mediterranean fever; chronic progressive multiple sclerosis; cirrhosis of the liver; biliary cirrhosis; Beh~et’s disease’, pseudogout (a condition caused by calcium deposits); amyloidosis; very low blood-platelet count (also known as ITP); skin reactions, including scleroderma, psoriasis, Sweet Syndrome, and other conditions; and nerve disability associated with chronic progressive multiple sclerosis.
General Information
While no one knows exactly how colchicine works, it appears to help people with gout by reducing the inflammatory response to uric acid crystals that form inside joints and by interfering with the body’s mechanism for making uric acid. Unlike drugs that affect uric acid levels, colchicine does not block the progression of gout to chronic gouty arthritis; it will, however, relieve the pain of acute attacks and lessen the frequency and severity of attacks. It has no effect on other kinds of pain.
Cautions and Warnings
Do not use colchicine if you are allergic or sensitive to any of its ingredients or you suffer from any serious blood, kidney, liver, stomach, or cardiac condition.
Vomiting, abdominal pain, diarrhea, nausea, kidney damage, and blood in the urine may occur with colchicine, especially at maximum doses. This can worsen existing gastrointestinal (GI) or other conditions. Stop taking the medication and call your doctor if you develop one of these symptoms.
She weakness that people develop while taking colchicine is frequently related to high levels of colchicine in the blood caused by poor kidney function and improves without treatment 3-4 weeks after the drug is stopped. This reaction is often mistaken for other conditions.
Periodic blood counts should be done if you are taking
colchicine for long periods of time.
Colchicine interferes with the absorption of vitamin B12 by af-
fecting the lining of the GI tract.
Colchicine may affect the process of sperm generation in men. The safety and effectiveness for use by children have not been
established.
Possible Side Effects
V Common: vomiting, diarrhea, and abdominal pain may occur if you take maximum doses of colchicine for an acute gout attack. You may also experience severe diarrhea, kidney and blood-vessel damage, blood in the urine, and reduced urination.
✓    Less common: hair loss, rash, appetite loss, and muscle and nerve weakness.
✓    Rare: with long-term colchicine therapy—reduced whiteblood-cell and platelet counts, nerve inflammation, blood-clotting problems, rash, unusual bleeding or bruising, tingling in the hands or feet, red or purple spots under the skin, and other reactions. Colchicine may interfere with sperm formation. Contact your doctor if you experience any side effect not listed above.
Drug Interactions
•    Colchicine interferes with the absorption of vitamin B12.
•    Colchicine may increase sensitivity to central-nervoussystem depressants, such as sedatives and alcohol.
•    The following drugs may reduce colchicine’s effectiveness: anticancer drugs, bumetanide, diazoxide, thiazide diuretics, ethacrynic acid, furosemide, mecamylamine, pyrazinamide, and triamterene.
•    Taking phenylbutazone with colchicine increases the risk of side effects.
•    Mixing the anitiiofic clarithromycin with colchicine can lead to colchicine toxicity, especially in the elderly and those with kidney disease.
Food Interactions None known.
Usual Dose
Acute Gout Attack: 1-1.2 mg. This dose may be followed by 0.51.2 mg every 1-2 hours until pain is relieved or nausea, vomiting, or diarrhea occurs. The total dose needed to control pain and in-
flammation during an attack varies from 4-8 mg.
Gout Prevention: 0.5-1.8 mg daily. In mild cases, 0.5 mg or 0.6 mg may be taken 3-4 days a week.
Familial Mediterranean Fever: 1-2 mg a day.
Cirrhosis of the Liver: 1 mg a day for 5 days each week. Biliary Cirrhosis: 0.6 mg twice a day.
Amyloidosis: 0.5 mg 1-2 times a day.
Behqet’s Disease: 0.5-1.5 mg a day.
Pseudogout: 0.6 mg twice a day.
ITP: 1.2-1.8 mg a day for 2 weeks or more.
Scleroderma: 1 mg a day.
Sweet Syndrome: 0.5 mg 1-3 times a day.
Other Skin Disorders: up to 1.8 mg a day, depending on the specific condition.
Overdosage
The lethal dose is estimated at 65 mg, although people have died after taking as little as 7 mg at once. Usually 1-3 days pass between the time that an overdose is taken and symptoms begin. Overdose symptoms start with nausea, vomiting, stomach pain, diarrhea—which may be severe and bloody—and burning sensations in the throat or stomach or on the skin. If you think you are experiencing overdose symptoms, contact your doctor immediately, or go to a hospital emergency room. ALWAYS bring the prescription bottle or container.
Special Information
Call your doctor if you develop rash, sore throat, fever, unusual bleeding or bruising, tiredness, weakness, numbness, or tingling. Seniors are move 4,ialy to develop drug side effects and should use this drug with caution.
Stop taking maximum doses of colchicine as soon as gout pain is relieved and reduce your dose to a maintenance level if your doctor has prescribed it for gout prevention. Stop taking the drug entirely and contact your doctor at the first sign of nausea, vomiting, stomach pain, or diarrhea.
If you forget a dose of colchicine, take it as soon as possible. If it is almost time for your next dose, skip the dose you forgot and continue with your regular schedule. Do not take a double dose.
Special Populations
Pregnancy/Breast-feeding: colchicine can harm the fetus. Pregnant women should not take it unless the potential benefits clearly outweigh the risks.
It is not known if colchicine passes into breast milk. No problems with nursing infants are known, but nursing mothers who must take colchicine should consider using infant formula.
Seniors: Seniors, especially those with renal, hepatic, gastrointestinal, or heart disease, are more likely to develop side effects and should use colchicine with caution.

General Information
Contraceptive drugs are synthetic hormones containing either progestin or a progestin-estrogen combination. The overall effects of any contraceptive are influenced by the interaction of all active ingredients, including those Huth -&))Otogenic and anti-estrogenic aqtk%(kkj.’%tte drugs are similar to natural female hormones, which cannot be used as contraceptives because very large dosages would be required. Synthetic hormones are more potent and are effective at smaller dosages. Contraceptive drugs work by preventing sperm from reaching the unfertilized egg, preventing the implantation of a fertilized egg in the uterus, or preventing ovula-tion (the release of an unfertilized egg from the ovaries). They prevent acne by balancing hormone levels.
When properly used, hormonal contraceptives can be 97-99% effective at preventing pregnancy. These products vary in their etfectiveness, and in the amount and type of estrogen or progestin used. The side effects of these drugs tend to increase with the amount of hormone they contain. While low hormone dosages are preferred, contraceptives with the smallest amounts of estrogen may be less effective in some women than others.
Single-phase products provide constant levels of estrogen and progestin throughout the entire month-long pill cycle. In 2-phase combinations, the amount of estrogen remains at a steady low level throughout the cycle, while progestin levels increase and then decrease. This variation in progestin allows normal changes to take place in the uterus. Three-phase products are meant to simulate the normal hormone cycle and reduce breakthrough bleeding. Throughout the cycle, estrogen levels remain the same while those of progestin change to create a 3-part wave pattern. The amount of estrogen in 3-phase products is considered low. Breakthrough bleeding may occur with the older combination products from day 8 through 16 of the cycle.
The mini-pill, a progestin-only product, may cause irregular menstrual cycles and may be less effective than estrogenprogestin combinations. Mini-pills may be recommended to older women or women who should avoid estrogens (see “Cautions and Warnings”).
The contraceptive patch releases small amounts of progestin and estrogen continuously over 3 weeks. The medication is absorbed into the blood vessels just below the skin. The patch works in the same way as contraceptive pills do.
The vaginal ring releases small amounts of etonogestrel, a progestin, and estradiol, an estrogen, in the vaginal canal over 3 weeks. The combination prevents pregnancy in the same way as combination pills do but may be less effective than contraceptive pills because some people find them harder to use.
Most contraceptive drugs are designed to simulate a normal Mt%VwM cycle. By not taking the hormones 1 week out of the month, you continue to have your regular period. In fact, these products are often used to stabilize a woman’s period. Two products, Seasonale and Seasonique, come in an 84-pill packet and are taken once a day for 3 months. This means you will only have your period once every 3 months. Another, Lybrel. is designed to be taken every day, eliminating monthly menstruation. Drosperinone, the progestin found in Yasmin and Yaz, has been found to relieve Premenstrual Syndrome (PMS) symptoms in addition to acting as an effective contraceptive hormone.
Levonorgestrel, a progestin, is used in implants that provide effective contraception for up to 5 years after surgical implantation under the skin of the upper arm or inside the uterus. Levonorgestrel implants should be replaced at least once every 5 years. Etonorgestrel implants are effective for 3 years. Implants can be removed at any time, reversing the contraceptive effect. The progestin intrauterine inserts provide effective contraception for about 1 year. The implant and intrauterine systems contain the same hormone found in the mini-pill and are associated with many of the same side effects and precautions as oral contraceptives.
Emergency contraceptives (sometimes referred to as the “morning-after pill”) contain high doses of estrogen and progestin. They are intended for use only after contraceptive failure or unprotected intercourse. They should never be taken by a pregnant woman.
Contraceptive drugs in any form are associated with risks. These risks are greatest in women over age 35 who smoke and have high blood pressure.
Cautions and Warnings
Do not take contraceptives if you are allergic or sensitive to them or any of their ingredients.
The risk of breast cancer may be slightly higher among current and recent users of combination oral contraceptives. This risk appears to decline after contraceptive use is stopped and is gone by 10 years after stopping combination contraceptive products. Breast cancers found in contraceptive users tend to be less advanced than those in non-contraceptive users.
You should not use contraceptive drugs if you are or might be pregnant, have had blood clots in veins or arteries, stroke, any blood-coagulation disorder, known or suspected cancer 0 the breast, sex organs, or liver. Products With more estrogen, or those t43.t jmmdr3higher sustained blood levels of estrogen, such as the contraceptive patch, are more likely to be associated with an increased risk of life-threatening blood clots.
Contraceptive drugs may cause eye lesions. Call your doctor at once if you develop visual difficulties of any kind.
Women taking the combination products Seasonale and Seasonique will have their period only once every 3 months and those taking Lybrel will not have a regular monthly period. It is absolutely essential for you to verify you are not pregnant if you think you may be pregnant for any reason.
The risks of contraceptive drugs increase if you are physically immobile or have asthma; cardiac insufficiency; epilepsy; migraine; kidney problems; a strong family history of breast cancer; benign breast disease; diabetes; endometriosis; gallbladder disease or gallstones; liver problems, including jaundice; high blood cholesterol; high blood pressure; estrogen or progestin intolerance; depression; tuberculosis; or varicose veins.
There is an increased risk of heart attack in women who have used contraceptive drugs for more than 5 years, or who are between age 40 and 49 and have other coronary risk factors such as smoking. obesity, high blood pressure, diabetes, and high blood cholesterol. This risk remains even after the medication is stopped.
Smokers in their mid-30s or older who use contraceptive drugs are 5 times more likely to have a heart attack than nonsmokers taking contraceptives and 10-12 times more likely to have a heart attack than nonsmokers who do not use the pill. Death due to circulatory disease also increases substantially in smokers taking contraceptive drugs, especially in women at least 35 years old. The risk of stroke is also increased in this group. Heavy smokers (more than 15 cigarettes a day) should not use hormonal contraceptives.
Women with a history of headaches, high blood pressure, or varicose veins should avoid estrogen-containing products, as Should older women and those who have experienced estrogen side effects.
Contraceptive drugs may mask the onset of menopause. Progestin-only products are associated with an increased risk of blood-clotting problems.
The progestin in Yasmin and Yaz raises blood potassium levels. Women with kidney, liver,,Dy adrenal gland disease should use eittlp_C pIrjdUcj with caution.
Intrauterine inserts have been associated with an increased risk of pelvic inflammatory disease (PID). The highest risk usually occurs within the first 20 days after insertion. Do not use intrauterine inserts if you have had an ectopic pregnancy.
Toxic Shock Syndrome has been associated with tampons, some barrier contraceptives, and the vaginal ring, although there is no proof that the product was the cause of the infection.
Possible Side Effects
♦ Common: Common side effects often result from using a product that is poorly suited to your body chemistry. Determining the right amount and type of hormone often minimizes these effects. If you are taking too much estrogen, you may experience nausea, bloating, high blood pressure, migraine, excess cervical mucous, skin discoloration, colon polyps, water retention, and swelling, or breast fullness or tenderness. Too little estrogen may cause early or mid-cycle breakthrough bleeding, spotting, or reduced periodic flow. Too much progestin is associated with weight gain and increased appetite, tiredness or fatigue, low periodic flow, acne, depression, breast regression, and androgen-related side effects (acne, oily scalp, hair loss, or excess hair growth). Too little progestin may cause late breakthrough bleeding, excessive periodic bleeding, or missed periods.
✓    Less common: abdominal cramps, infertility after discontinuance of the drug, breast tenderness, weight change, headache, rash, vaginal itching and burning, general vaginal infection, nervousness, dizziness, depression, cataracts, changes in sex drive, hair loss, and increased sensitivity to the sun.
✓    Rare: Women who use contraceptive drugs are more likely to develop several serious conditions, including blood clots in the deep veins, stroke, heart attack, liver cancer, gallbladder disease, and high blood pressure. Women who smoke cigarettes are at much higher risk for some of these adverse effects. Contact your doctor if you experience any side effect not listed above.

•    Ampicillin, barbiturates, bexarotene, bosentan, carbamazepine, chloramphenicol, efavirenz, fluconazole, griseofulvin, ketoconazole, neomycin, nelfinavir, nitrofuratoin, oxcarbazepine, phenylbutazone, phenytoin, penicillin drugs, protease inhibitor drugs for HIV, rifampin, rifapentine, statin drugs (atorvastatin and rosuvastatin), St. John’s wort, sulfa drugs, tetracycline products, and sedatives can make all contraceptive drugs less effective. Use backup birth control while taking these medications together.
•    Contraceptive drugs may elevate blood levels of benzodiazepine sedatives and sleeping pills (midazolam, lorazepam, oxazepam, and temazepam), caffeine, cyclosporine, imatinib, metoprolol, corticosteroids, theophylline drugs, tizanidine, triptan-type migraine drugs, and tricyclic antidepressants, increasing the risk of side effects. Discuss mixing these medicines with your doctor. Dosage reductions may be needed.
•    Contraceptive drugs may increase the toxic liver effects of acetaminophen and reduce the drug’s effectiveness. Contraceptive drugs may increase or decrease the effect of anticoagulant (blood-thinning) drugs. Discuss the risks of this combination with your doctor.
•    Mycophenolate interferes with only those contraceptives that contain levonorgestrel (Alesse, Aviane, Lessina. Levora, Levlite, Lutera, Lybrel, Mirena, Nordette, Norplant II, Portia, Plan B, Seasonale, Seasonique, and Triphasil). Backup contraception is recommended.
•    Exenatide may reduce the effectiveness of contraceptive pills.
Take them at least 1 hour before an injection of exenatide.
•    Contraceptive drugs may reduce the effectiveness of clofibrate for elevated blood triglycerides, sulfonylurea drugs for diabetes, ursodiol for gallbladder disease, and pain relievers, including salicylates (aspirin).
•    Contraceptive drugs may increase blood-cholesterol levels and interfere with blood tests for thyroid function and blood sugar.
•    Acetaminophen may increase blood levels of ethinyl estradiol, a common contraceptive drug ingredient, increasing side effects and reducing contraceptive effectiveness.
•    Since Yasmin and Yaz raise blood potassium levels, nether should be used if you ate taking spironolactone or anD)ftr potassium-sparing diuretic, potassium supplements, angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor antagonists, aldosterone antagonists, heparin, non-steroidal anti-inflammatory drugs (NSAIDs), or other medications on a long-term basis that may further increase potassium levels.
•    Contraceptive drugs may interfere with the effects of insulin
for diabetes.
•    Acitretin interferes with the contraceptive effect of progestin-
only mini-pills. It is not known if it also interferes with combination contraceptive drugs.
Food Interactions
None known. Usual Dose
Single-Phase, 2-Phase, and 3-Phase Combinations: The first day of bleeding is day 1 of the menstrual cycle. Beginning on the first day of the cycle, take 1 pill a day for 20-21 days according to the number of pills supplied by the manufacturer. If menstrual flow has not begun 7 days after taking the last pill, begin the next month’s cycle of pills. Some manufacturers recommend starting the pills on a Sunday to make it easy to remember to take them. In this case, start taking your pills on the first Sunday after your period begins. If menstruation begins on a Sunday, take the first pill that day.
Seasonale: Take 1 pink tablet every day for 84 consecutive days. Do not skip a day. Then, take 1 white pill a day for 7 days. Then, start a new pill cycle. You may be pregnant if you do not have a period while you are taking the white pills.
Seasonique: Take 1 light blue-green tablet containng levonorgestrel and ethinyl estradiol daily for 84 consecutive days, followed by 7 days of ethinyl estradiol tablets. Do not stop if spotting or breakthrough bleeding occurs. Report prolonged bleeding to your doctor.
Progestin-Only Mini-Pill: Take 1 pill every day.
Contraceptive Patch: Apply a new patch to the thigh, abdomen, or arm. Remove the patch after 3 weeks and then reapply a new patch after 1 week. Be sure to always apply a new patch on the same day of the week. If you are switching from birth control pmts, apply the first patch on the same day you would start a new cycle of pills.
Vaginal Ring: Keep the vaginal ring in the vaginal canal for 3 weeks. Remove it and put a new one in 1 week later. If you did not use a hormonal contraceptive in the previous month, insert the ring between day 1 and day 5 of your cycle.
If you are switching from a combination birth control pill, insert the ring anytime during the week after you took your last pill but before you would have started your next cycle of pills. No additional contraception is necessary.
If you are switching from a mini-pill, insert the ring on the day after you take your last mini-pill.
If you are switching from a progestin implant or an IUD, insert the ring on the same day your implant or IUD is removed.
If you are switching from a progestin injection, insert the ring on the same day you would have received your next injection.
If you are switching from a progestin-only mini-pill, implant, injection, or IUD, use another form of contraception for the first 7 days after you insert the ring.
Emergency Contraception: Emergency contraceptive kits have only a few pills. They should be taken with a full meal. Take half the pills (1 or 2 depending on the brand you use) within 72 hours of unprotected sex, however they are most effective when taken within the first 24 hours. Take the rest of the pills 12 hours after the first dose. Emergency contraceptives reduce the risk of pregnancy by 75%.
The pregnancy test in the kit can be used to determine if you became pregnant earlier in your cycle or during a previous cycle. If the test is positive, consult your doctor before taking emergency contraception. If you vomit within one hour of taking either dose, contact your doctor.
Overdosage
An overdose may cause nausea and withdrawal bleeding in adult women. Overdose victims should be taken to a hospital emergency room. ALWAYS bring the prescription package.
Special Information
Use backup birth control to prevent pregnancy in the first 3 weeks after you begin taking contraceptive drugs.
Contraceptive drugs do not protect against sexually transmitted diseases.
VM your pill at the same time each day to establish a routine and ensure maximum contraceptive protection.
Call your doctor immediately if you experience severe abdominal pain; severe or sudden headache; pain in the chest, groin, or leg, especially the calf; sudden slurring of speech; changes in vision; weakness, numbness, or pain in the arms or legs; coughing up of blood; loss of coordination; or shortness of breath. These symptoms may require emergency treatment.
Other problems that may require medical attention are bulging eyes; changes in vaginal bleeding; fainting; frequent or painful urination; a gradual increase in blood pressure; breast lumps or secretions; depression; yellowing of the skin or whites of the eyes; rash; redness or irritation; upper abdominal swelling, pain, or tenderness; an unusual or dark-colored mole; thick, white vaginal discharge; or vaginal itching or tenderness.
See your doctor for a check-up every 6-12 months.
Some manufacturers include 7 inert or iron pills in their packaging to be taken on days when the drug is not taken. This makes it easier for women to stay on schedule with their pills. The 7 pills bridge the gap between contraceptive cycles and allow women to take 1 pill every day without stopping.
For single- or 2-phase combinations: If you forget to take a pill for 1 day, take 2 pills the following day. If you miss 2 consecutive days, take 2 pills for the next 2 days. Then return to your schedule of 1 pill a day. If you miss 3 consecutive days, do not take any pills for the next 7 days and use another form of contraception; then start a brand new cycle.
Seasonale: The risk of pregnancy increases with each pink tablet you forget. Use another method of non-hormonal backup contraception any time you miss 2 or more pink tablets until you have taken a pink tablet every day for 7 consecutive days. You are protected against pregnancy if you miss 1 or more white tablets, as long as you begin taking the pink tablets again on the proper day.
Seasonique: The risk of ovulation and pregnancy increases with each forgotten light blue-green pill. If you miss 1 light blue-green pill, take it as soon as you remember and take the next pill at your regular time. This may mean you will take 2 pills on the same day. You don’t need to use a backup birth control method if you forget only 1 pill. If you forget 2 light blue-green pills in a row, take 2 pills on the day you remember and take 2 pills the ReYA day. -Then go back to taking 1 pill a day MMI you finish your pack. If you miss 2 01 MbreYight blue-green pills in a row, you must use non-hormonal backup contraception until you have taken a light blue-green pill daily for 7 days in a row. If you miss 1 or more yellow tablets, you are still protected against pregnancy provided you begin taking light blue-green pills again on the proper day.
For 3-phase combinations: If you forget to take a pill for 1 day, take 2 pills the following day. If you miss 2 consecutive days, take 2 pills for the next 2 days. Then return to your schedule of 1 pill a day. If you forget to take a pill for 3 days in a row, stop taking the drug and use an alternate means of contraception until your period starts. ALWAYS use a backup contraceptive method for the remainder of your cycle if you forget even 1 pill of a 3-phase combination.
If you forget to apply the contraceptive patch on the same day of the week once every 4 weeks, you risk a loss of effectiveness on the days after you should have applied it. If the patch comes off or is partially detached in mid-cycle, you must start a new 3-week cycle at once by removing the old patch and applying a new one.
If the vaginal ring is accidentally expelled during the 3 weeks it is normally retained, rinse it off with water and replace it within 3 hours. Do not use hot water. If the ring is not reusable, insert a new ring and continue with your regular schedule. If you do not replace the ring within 3 hours, its effectiveness may be reduced. If you are in week 3 of the cycle, throw the ring away; you may insert a new one immediately, which will begin a new 3-week cycle and cause you to skip a period. Or, you may wait a week, during which time you will have periodic bleeding, and insert a new ring no later than 7 days after the vaginal ring was expelled. This option should be chosen if you had used the ring for 7 days in a row before it was expelled.
If, when it is expelled, you are in week 1 or 2 of your cycle and the ring is out for more than 3 hours, reinsert it and use an additional form of contraceptive until the ring has been worn for 7 consecutive days. A vaginal ring may break and then slip out or cause discomfort. Throw the ring away if this happens.
Missing a pill reduces your protection. If you keep forgetting to take your pills, you must use another birth control method.
If you take drugs that reduce the effectiveness of contraceptive drugs (see “Drug Interactions”), use a backup contraceptive method during that cycle to PMNeiA accidental pregnancy.
GOOD tlL:Inlal hygiene is essential while taking contraceptive drugs. See your dentist regularly and brush and floss carefully because contraceptive drugs may increase the risk of an oral infection.
Contraceptive drugs may increase your sensitivity to the sun.
Wearing contact lenses may be uncomfortable while taking contraceptive drugs because the pills can cause minor changes in the shape of your eyes.
All contraceptive prescriptions come with a “patient package insert.” Read it thoroughly as it gives detailed information about the drug and is required by federal law.
Special Populations
PregnancylBreast-feeding., Contraceptive hormones cause birth defects and may interfere with fetal development. They are not safe for use during pregnancy. If you think you are pregnant, use another form of contraception and stop taking your birth control pills.
Contraceptive hormones pass into breast milk. Combination contraceptive products reduce the amount of milk produced. Nursing mothers who must use any of these drugs should use infant formula.

General Information
clotrimazole is useful against a variety of fungal organisms that other drugs do not affect. The exact way in which clotrimazole works is unknown. Sertaconazole is used for athlete’s foot in people age 12 and older with compromised immune systems.
Cautions and Warnings
Do not use this product if you are allergic or sensitive to any of its ingredients.
If clotrimazole causes local itching or irritation, stop using it. Do not use clotrimazole in your eyes.
Proper diagnosis is essential for effective treatment. Do not use this product without first consulting your doctor.
Possible Side Effects
Side effects are infrequent and usually mild.
Cream and Solution
V Most common: redness, stinging, blistering, peeling, itching, and swelling of local areas.
Vaginal Tablets
♦ Most common: mild burning, rash, mild cramps, and frequent urination. Your sexual partner may also experience some burning or itching.
Lozenges
V Most common: stomach cramps or pain, diarrhea, nausea, and vomiting.

Food %%ractions
The oral form of clotrimazole is best taken on an empty stomach, at least 1 hour before or 2 hours after meals. However, you may take it with food as long as you allow the lozenge to dissolve fully in your mouth.
Usual Dose
Topical Cream and Solution
Adult and Child (over age 2): Apply to clean, dry, affected areas morning and night for 7 consecutive days or as needed. For athlete’s foot and ringworm, use daily for 4 weeks. For jock itch, use daily for 2 weeks.
Vaginal Cream
Adult: 1 applicator’s worth at bedtime for 3-7 consecutive days.
Vaginal Tablet
Adult: 1 tablet inserted into the vagina at bedtime for 3 days, or 2 tablets a day for 3-7 consecutive days.
Lozenge
Adult and Child (over age 3): 1 lozenge 5 times a day for 2 weeks or more.
Overdosage
Little is known about the effects of clotrimazole overdose or accidental ingestion. Call your local poison control center for more information. If you seek treatment, ALWAYS bring the prescription bottle or container.
Special Information
If treating a vaginal infection, you should refrain from sexual activity. Call your doctor if burning or itching develops or if the condition does not improve within 7 days.
If you are using the vaginal cream, you may want to wear a sanitary napkin to avoid staining your clothing. Do not use a tampon during treatment.
Dissolve the lozenge slowly in the mouth. This may take up to 30 minutes.
This medicine must be taken on consecutive days. If you forget a dose of oral clotrimazole, take it as soon as you remember. Do not double your dose.
When using clotrimazole for skin infections, do not cover the area with any kind of bandage unless directed to do so by your doctor. For athlete’s foot, wear well-fitting, ventilated shoes, and change your socks at least once a day.
clotrimazole is not effective on scalp or nails.
Special Populations
Pregnancy/Breast-feeding: Women who are or might be pregnant should talk to their doctor about the medication’s risks and benefits. Women who are in the first 3 months of pregnancy should use this drug only if directed to do so by their doctor. If you are pregnant, your doctor may want you to insert vaginal tablets by hand rather than use a vaginal applicator.
It is unknown whether the drug passes into breast milk. Use with caution or use infant formula.
Seniors: Seniors may use this medication without special precaution.

General Information
Clozapine is a unique antipsychotic that has the capacity to treat people who do not respond to or cannot tolerate other drugs. It works by a mechanism that differs from those of other antipsychotic drugs.
A very small number of people who take clozapine develop a rapid drop in their white-blood-cell count, known as agranulocytosis. This effect usually reverses itself when the drug is stopped, but the drug must be stopped as soon as it is discovered. An unusually large number of people who have developed clozapine algllaTwlocytosis in the United States are of Eastern European Jewish descent, but the association is not very strong. Most cases of agranulocytosis occur between week 4 and week 10 of treatment. It is essential that blood samples be taken approximately every week and for 4 weeks after the drug is stopped to watch for this effect. Because of the risk of agranulocytosis, clozapine should not be tried until at least 2 other antipsychotic medicines have failed.
Some people taking antipsychotic drugs develop tardive dyskinesia, a potentially irreversible condition marked by uncontrollable movements. Tardive dyskinesia has not been seen in patients taking clozapine, a major advantage of this drug over other antipsychotic medicines. However, there is still a risk that this set of symptoms could occur with clozapine.
Cautions and Warnings
Do not take clozapine if you are allergic or sensitive to any of its ingredients.
Women, seniors, people with serious illnesses, those who are emaciated. those with a history of diseases affecting the white blood cells, or those who are taking other medication that could affect white blood cells may be more susceptible to clozapine agranulocytosis.
Clozapine has been associated with increased mortality in seniors with dementia or Alzheimer’s disease. The specific causes of death related to clozapine and other atypical antipsychotic drugs were either due to a heart-related event or infection, mostly pneumonia. Clozapine should not be taken by those with dementia-related psychosis.
About 5% of people taking the drug experience a seizure in the first year of treatment. Seizure is most likely to occur at higher drug doses.
People with heart disease should be carefully monitored while on clozapine because of possible cardiac risks.
Clozapine may cause low blood pressure, especially at the beginning of therapy.
Clozapine has been associated with obesity, high cholesterol, high blood sugar, and diabetes. Diabetics and pre-diabetics (people with elevated blood sugar and a family history of diabetes) should be carefully monitored.
A serious set of side effects, known as neuroleptic malignant syndrome (NMS), includes a high lever and has been associated With clozapine when it is used together with lithium or other drugs. The symptoms that constitute NMS include muscle rigidity, mental changes, irregular pulse or blood pressure, increased sweating, and abnormal heart rhythm. NMS is potentially fatal and requires immediate medical attention.
Use this drug with caution if you have glaucoma, prostate
problems, or liver or kidney disease.
clozapine may interfere with mental or physical abilities because of the sedation it usually causes during the first few weeks
of treatment.
Possible Side Effects
✓    Most common: rapid heartbeat, low blood pressure, dizziness, fainting, drowsiness or sedation, salivation, and constipation.
✓    Less common: headache, tremor, sleep disturbance, restlessness, slow muscle motions, absence of movement, agitation, convulsions, rigidity, restlessness, confusion, sweating, dry mouth, visual disturbances, high blood pressure, nausea, vomiting, heartburn or abdominal discomfort, fever, and weight gain.
♦    Rare: agranulocytosis (symptoms include fever with or without chills, sore throat, and sores or white spots on the lips or mouth), tardive dyskinesia (symptoms include lip smacking or puckering, puffing of the cheeks, rapid or wormlike tongue movement, uncontrolled chewing motions, and uncontrolled arm and leg movements), and NMS (see “Cautions and Warnings”). Other rare side effects can occur in almost any part of the body. Contact your doctor if you experience any side effect not listed above.
Drug Interactions
•    Clozapine’s anticholinergic effects—blurred vision, dry mouth, and confusion—may be enhanced by interaction with other anticholinergics, such as tricyclic antidepressants like amitriptyline.
•    Drugs that reduce blood pressure may enhance the bloodpressure-lowering effects of clozapine.
•    Alcohol and other nervous system depressants, including benzQUIQOmrn and other antianxiety drugs, may enhance clozapine’s sedative action. At least 1 person has died as a result of combining diazepam and clozapine.
•    Combination contraceptive drugs may increase blood levels of clozapine leading to toxic side effects. Women starting on a combination contraceptive may need to have their clozapine dose adjusted.
•    Clozapine should not be used with ritonavir.
•    Cimetidine, caffeine, citalopram, ciprofloxacin, erythromycin, and ketoconazole may increase blood levels of clozapine resulting in increased side effects. Caution should be used with combining clozapine with paroxetine, fluvoxamine, or sertraline as similar reactions may occur, although these interactions are less well-defined.
•    Clozapine may increase blood levels of digoxin, warfarin, heparin, and phenytoin.
•    Use of clozapine with phenytoin, carbamazapine, and rifampin may cause decreases in blood levels of clozapine, reducing its effectiveness.
•    The combination of lithium and clozapine may cause seizures, confusion, and NMS (see “Cautions and Warnings”).
•    Cigarette smoking may alter clozapine dosage requirements.
•    Combining selective serotonin receptor inhibitors (SSRls) with clozapine may require a lower clozapine dosage.
Food Interactions None known.
Usual Dose
Tablets
Starting dose: 25 mg in divided doses twice a day; maintenance dose    generally, 300-450 mg a day in divided doses. Dosage may be increased gradually to a daily maximum of 900 mg in divided doses if required.
Orally Disintegrating Tablets
Starting dose: 12.5 mg once or twice a day increasing to 300450 mg a day in divided doses by the end of 2 weeks. Dosage may then be increased up to 900 mg a day in divided doses if required.
Overdosage
Symptoms of overdose are delirium, drowsiness, changes in heart rhythm, unusual excitement, nervousness, restlessness, hallucinations, excessive salivation, dizziness or fainting, slow or irregular breathing, and coma, Overdose victims must be taken to a hospital emergency room immediately. Special Information
Clozapine may cause a fever during the first few weeks of treatment. Generally, the fever is not important, but it may occasionally be necessary to stop treatment due to a persistent fever.
Regular blood tests are necessary to monitor blood composition for any changes that might be caused by clozapine.
Call your doctor at once if you develop lethargy or weakness, a flu-like infection, sore throat, feelings of ill health, fever, sweating, muscle rigidity, mental changes, irregular pulse or blood pressure, mouth ulcers, or dry mouth that lasts for more than 2 weeks.
Dry mouth, a common side effect of clozapine, may be countered by using gum, candy, ice, or a saliva substitute such as Orex or Moi-Stir.
Do not stop taking clozapine without your doctor’s knowledge and approval, because a gradual dosage reduction may be necessary to prevent side effects.
Avoid alcohol or any other nervous system depressants while taking clozapine.
Some of the side effects of clozapine    drowsiness, blurred vision, and seizures—may interfere with the performance of complex tasks like driving or operating hazardous equipment.
While taking clozapine, rapidly rising from a sitting or lying position may cause you to become dizzy or faint.
If you take clozapine twice a day and forget a dose, take it as soon as you remember. If it is almost time for your next dose, take 1 dose as soon as you remember and another in 5 or 6 hours, then go back to your regular schedule. If you take clozapine 3 times a day and forget a dose, take it as soon as you remember. If it is almost time for your next dose, take 1 dose as soon as you remember and another in 3 or 4 hours, then go back to your regular schedule. Never take a double dose.
Orally disintegrating tablets should be left in the unopened blister until time of use. They should not be pushed through the foil. Just prior to use, peel the foil from the blister and gently remove the orally disintegrating tablet. Immediately place the tablet in the mouth, allow it to disintegrate and then swallow with saliva. No water is needed.
Special Populations
Pregnancy/Breast-feeding: This drug Should be used during PM Only if your doctor determines that it is absolutely necessary.
clozapine may pass into breast milk. Nursing mothers who must take this drug should use infant formula.
Seniors: Seniors may be more sensitive to the side effects of clozapine, such as dizziness on rapidly rising from a sitting or lying po-sition, confusion, and excitability. Older men are also more likely to have prostate problems, a reason to be cautious with clozapine. Seniors with psychosis due to dementia who take clozapine are more likely to die from heart disorders and infections than those not taking it.

Prescribed For
Mild to severe pain, breakthrough cancer pain, and cough. Long-acting narcotics are meant only for people with chronic pain. Also prescribed for pain and anxiety in pediatric burn patients.
General Information
Codeine relieves pain and suppresses cough. The pain-relieving effect of 30-60 mg of codeine is equal to approximately 650 mg, or 2 tablets, of aspirin. Codeine may be less effective than aspirin for pain associated with inflammation because aspirin reduces inflammation and codeine does not. Codeine suppresses the cough reflex but does not cure the underlying cause of the cough. Other narcotic cough suppressants are stronger pain relievers, but codeine remains the best cough medication available.
Morphine sulfate is a pure narcotic that has been in use for many years. In addition to pain relief, morphine’s effects include drowsiness, mood changes, breathing difficulty, slowed movement of the gastrointestinal tract, nausea, vomiting, and changes in the endocrine and autonomic nervous systems. Morphine sulfate liquid, immediate-release tablets, and suppositories must be taken several times a day. The medication they contain is released immediately for absorption into the bloodstream. Extended- and controlled-release morphine products are designed to release some of the narcotic right away and the rest over a 24-hour period, allowing for less-frequent dosage.
Fentanyl is a potent pain reliever that can be substituted for other narcotic drugs. The patch form, which must be replaced about every 3 days, delivers fentanyl to the bloodstream at a steady rate. The lozenge has a shorter length of action than any other narcotic pain reliever, which makes it useful when given to children before surgery because it provides doctors with the flexibility to obtain maximum benefit with minimal side effects. The lozenge on a stick is used for breakthrough cancer pain as a booster for people already taking narcotic pain relievers. These forms should only be used under controlled circumstances because of the risk of side effects or overdose. Low dosages of fentanyl relieve pain—larger amounts cause loss of consciousness and breathing difficulties.
Oxycodone is a narcotic used to control moderate to severe pain. Most people take it together with aspirin (Percodan) or acetaminophen (Percocet), but it can be used by itself. This is a potent pain reliever that carries a risk (31 addiction with continued use.
Cautions and Warnings
Do not take narcotics if you are allergic or sensitive to any of their ingredients.
Long-term use of narcotics may cause drug dependence or addiction.
Use narcotics with extreme caution if you suffer from asthma or other breathing problems.
Narcotics may make it difficult to monitor the progress of people who have suffered head injuries and acute abdominal conditions.
Actiq contains fentanyl in an amount that can be fatal to children. Keep used and unused lozenges and lozenges on a stick out of reach of children.
Possible Side Effects
♦    Most common: lightheadedness, dizziness, sleepiness, nausea, vomiting, appetite loss, and sweating. If these occur, ask your doctor about lowering your dosage. Most of these side effects disappear if you lie down.
♦    Less common: euphoria (feeling “high”), headache, agitation, uncoordinated muscle movement, minor hallucinations, disorientation and visual disturbances, dry mouth. constipation, flushing of the face, rapid heartbeat, palpitations, faintness, urinary difficulties or hesitancy, reduced sex drive or impotence, itching, rash, anemia, lowered or raised blood sugar, and yellowing of the skin or whites of the eyes. Narcotic analgesics may aggravate convulsions in those who have had them.
More serious side effects of codeine are shallow breathing or breathing difficulties.
Drug Interactions
•    Avoid combining narcotics with alcohol, sleeping medications, sedatives, other depressant drugs, or non-prescription drugs that have alcohol as an ingredient. Alcohol speeds the release of morphine from Avinza. The mixture can result in a deadly narcotic overdose.
•    Narcotic analgesics should not be used at the same time as monoamine oxidase inhibitor antidepressants. Separate usage by at least 14 days.
•    Combining a narcotic pain reliever with an anticholinergic medication may result in severe constipation.
•    Combining a narcotic pain reliever with any other medication that lowers blood pressure can lead to excessive blood-pressure lowering. Avoid this combination.
•    Combining cimetidine with a narcotic pain reliever may cause confusion, disorientation, breathing difficulties, and seizure.
•    Reserpine, rifampin, and remifentanil may decrease the pain-relieving effects of morphine.
•    Fentanyl should be used with caution with azole antifungals (e.g. ketoconazole).
Food Interactions
Codeine may be taken with food to reduce upset stomach. Morphine capsules and the fentanyl patch may be used without regard to food.
Usual Dose
Dosing of narcotic pain medications is highly individualized based on patient tolerance and response to medication.
Codeine
Adult: 15-60 mg every 4-6 hours for relief of pain; 10-20 mg every few hours as needed to suppress cough.
Child: 1 mg per lb. of body weight every 4-6 hours for relief of pain; 2.5-10 mg every 4-6 hours to suppress cough.
Fentanyl Lozenge and Lozenge on a Stick
Adult: 200-1600 mcg. Dosage may be repeated up to 4 times daily. Allow the lozenge to dissolve in your mouth. DO NOT CHEW. Child: not recommended.
Fentanyl Patch: Apply to a clean and non-irritated patch of skin as directed, usually once every 3 days.
Morphine Extended-release and Controlled-release
Tablets and Capsules
Adult: 1-3 capsules a day, depending on the specific product and individual need.
Morphine Oral Liquid and Immediate-release Tablets Adult: 5-30 mg every 4 hours.
Morphine Suppositories
Adult: 5-30 mg several times a day.
Oxycodone
Adult: 10-30 mg every 4 hours as needed. OxyContin should be swallowed whole and not broken.
Child: not recommended.
Overdosage
Symptoms include breathing difficulties or slowing of respiration, extreme tiredness progressing to stupor and then coma, pinpointed pupils, no response to pain stimulation, cold and clammy skin, slowing of heartbeat, lowering of blood pressure, convulsions, and cardiac arrest. The victim should be taken to a hospital emergency room immediately. ALWAYS bring the prescription bottle or container.
Special Information
Codeine is a respiratory depressant and affects the central nervous system (CNS), producing sleepiness, tiredness, or inability to concentrate. Be careful when driving or doing any task that requires concentration. Avoid alcohol.
Call your doctor if you develop breathing difficulties, constipation, dry mouth, or any bothersome or persistent side effect.
Apply the fentanyl patch only to non-irritated skin on a flat surface of the upper body. Hair at the application site should be clipped or cut, not shaved, before applying the patch. Do not use oils, soaps, lotions, alcohol, or anything else that might irritate the skin before applying the patch.
If you are taking a controlled-release narcotic product, do not crush, chew, or break the tablet or lozenge. Rapid release may result in a potentially fatal dose of the drug.
If you forget a dose of codeine, take it as soon as you remember. If it is almost time for your next dose, skip the one you forgot and continue with your regular schedule. Never take a double dose.
Special Populations
Pregnancy/Breast-feeding: Narcotics pass into the fetal circulation. Excessive use of them during pregnancy may cause drug dependence in newborns. Narcotics may also cause breathing difficulties in infants during delivery. Animal studies show that codeine may cause fetal harm. If given to a pregnant woman before cesarean section, fentanyl may cause drowsiness in newborns. When either of these drugs is considered crucial by your doctor, its potemt(a1 bel)elft must be carefully weighed against its risks.
Narcotics pass into breast milk. Nursing mothers who must take codeine should use infant formula.

Prescribed For
High blood pressure, including hypertensive emergency (diastolic blood pressure over 120); also used for excess sweating, childhood growth delay, attention-deficit hyperactivity disorder (ADHD), Tourette’s syndrome, restless leg syndrome, schizophrenic psychosis, migraine, ulcerative colitis, painful or difficult menstruation, hot flashes related to menopause, diagnosis of pheochromocytoma (adrenal-gland tumor), kidney poisoning associated with cyclosporine, diabetic diarrhea, smoking cessation, methadone and opiate detoxification, withdrawal from alcohol and benzodiazepines such as Valium, nerve pain following herpes attack, and allergic reactions in the presence of asthma triggered by external sources.
General Information
Clonidine stimulates nerve endings in the brain called alphaadrenergic receptors. It reduces blood pressure by dilating (widening) blood vessels. Clonidine works quickly, decreasing blood pressure within 1 hour. The other uses of clonidine relate to its stimulation of alpha receptors in the body.
Cautions and Warnings
Do not take clonidine if you are allergic or sensitive to any of its ingredients.
People who have had a stroke or recent heart attack or who have cardiac insufficiency or chronic kidney failure should avoid taking clonidine.
Some people develop a tolerance of their clonidine dosage. If this happens, your blood pressure may increase and your doctor may prescribe a higher dose.
Never stop taking clonidine without your doctor’s knowledge. If you abruptly stop taking clonidine, you may experience an unusual increase in blood pressure accompanied by agitation, headache, nervousness, and severe reactions, possibly death. Restarting clonidine therapy or taking another antihypertensive can reverse these effects.

People who develop skin sensitivity (symptoms include rash, itching, and swelling) to Catapres-TTS, the transdermal patch form of clonidine, may experience the same reactions with oral clonidine.
Possible Side Effects
Tablets
♦    Most common: dry mouth, drowsiness, dizziness, constipation, and sedation.
♦    Common: headache and fatigue. These effects tend to diminish within 4-6 weeks.
•    Less common: appetite loss, swelling or pain in the glands of the throat, nausea, vomiting, weight gain, blood-sugar elevation, breast pain or enlargement, worsening of congestive heart failure, heart palpitations, rapid heartbeat, painful blood-vessel spasm, abnormal heart rhythms, electrocardiogram changes, feeling unwell, changes in dream patterns, nightmares, difficulty sleeping, hallucinations, delirium, anxiety, depression, nervousness, restlessness, rash, hives, thinning or loss of scalp hair, difficult or painful urination, nighttime urination, retaining urine, decrease or loss of sex drive, weakness, muscle or joint pain, leg cramps, increased alcohol sensitivity, dryness and burning of the eyes, dry nose, loss of color, and fever.
Transdermal Patch
✓    Most common: dry mouth and drowsiness.
✓    Less common: constipation, nausea, changes in sense of taste, dry throat, fatigue, headache, lethargy, changes in sleep patterns, nervousness, dizziness, impotence, sexual difficulties, and mild skin reactions including itching, swelling, contact dermatitis, discoloration, burning, peeling, throbbing, white patches, and generalized rash. Rashes of the face and tongue have also occurred but cannot be specifically tied to transdermal clonidine.
Drug Interactions
•    Combining clonidine and a beta-adrenergic blocker may increase the severity of a drug-withdrawal reaction and rebound high MW pressure. This reaction may be very serious.
•    Combining verapamil and clonidine may lead to very low blood pressure and atrioventricular (AV) block (abnormality in heartbeat patterns). This reaction may be very serious.
O    Avoid alcohol, barbiturates, and sedatives because they increase the depressive effects of clonidine.
•    Tricyclic and other antidepressants, appetite suppressants, estrogens, stimulants, indomethacin and other nonsteroidal anti-inflammatory drugs (NSAIDs), and prazosin may counteract the effects of clonidine.
•    clonidine may reduce the therapeutic effects of levadopa +
carbidopa.
Food Interactions
The tablets are best taken on an empty stomach but may be taken with food if they upset your stomach.
Usual Dose
Tablets
Adult: high blood pressure-100 mcg twice a day to start; may be raised by 100 mcg a day until maximum control is achieved. Take no more than 2400 mcg a day. Other uses-100-goo mcg a day, or up to 0.8 mcg per lb. of body weight in divided doses. Seniors should start with a lower dose and increase more slowly.
Child: 50-400 mcg orally twice a day.
Transdermal Patch
Adult: 100 mcg delivered daily from a patch applied once every 7 days. Up to two 300-mcg patches may be needed to control blood pressure. Transdermal dosage exceeding 600 mcg a day has not been shown to increase effectiveness.
Child: not recommended.
Overdosage
Symptoms of overdose are slow heartbeat, central- nervous-system depression, very slow breathing, low body temperature, pinpoint pupils, seizures, lethargy, agitation, irritability, nausea, vomiting, abnormal heart rhythms, mild increases in blood pressure followed by a rapid drop in blood pressure, dizziness, weakness, loss of reflexes, and vomiting. Victims should be taken to a hospital emergency room immediately. ALWAYS bring the prescription bottle or container.
Special Information

Call your doctor it you become depressed or have vivid dreams or nightmares while taking clonidine, or if you develop swelling in your feet or legs, paleness or coldness in your fingertips or toes, or any persistent or bothersome side effect.
Apply the transdermal patch to a hairless area of skin such as the upper arm or torso. Use a different skin site each time. If the patch becomes loose, apply the supplied adhesive directly over it. If the patch falls off before 7 days are up, apply a new one. Do not remove the patch while bathing.
If you forget a dose of oral clonidine, take it as soon as possible and then go back to your regular schedule. If you miss 2 or more consecutive doses, consult your doctor; missed doses may cause blood pressure increases and severe adverse effects. Do not take a double dose.

General Information
Artery-clogging blood clots are often the cause of heart attacks and strokes. clopidogrel reduces the risk of both by helping prevent blood-clot formation. This drug thins the blood by making platelets—the cells that aggregate to form clots—less “sticky.” It starts working in as little as 2 hours after taking a single tablet. The drug’s blood-thinning effect lasts until inactivated platelets are replaced by the body. Studies suggest that clopidogrel is more effective than aspirin in preventing heart attack and stroke in people at risk. People taking clopidogrel after scent surgery usually take it for a relatively short period. Those taking it to prevent a heart attack or stroke must take it for life.
Cautions and Warnings
Do not take clopidogrel if you are allergic or sensitive to any of its ingredients or to ticlopidine, a related antiplatelet. These drugs can rarely cause a rapid drop in white-blood-cell count.
People with bleeding ulcers, brain hemorrhages, or other bleeding problems should use clopidogrel with caution.
Thrombotic thrombocytopenic purpura (TTP) is a rare but serious complication of clopidogrel, sometimes reported after less than 2 weeks of treatment. See your doctor right away if you develop a sudden fever, unusual bruising, nosebleeds, bleeding gums, or any other unusual symptoms. TTP reduces your platelet count, interfering with blood clotting, and affects white-blood-cell count.
People with liver problems should use clopidogrel with caution.
Possible Side Effects
✓    Most common: rash and other skin problems.
✓    Common: chest pain, accidents, flu-like symptoms, pain, headache, dizziness, abdominal pain, upset stomach, joint pain, back pain, black-and-blue marks, and respiratory infection.

Drug Interactions
•    Clopidogrel may interfere with the body’s ability to break down fluvastatin, nonsteroidal anti-inflammatory drugs (NSAIDs), phenytoin, tamoxifen, tolbutamide, torsamide, and
warfann.
•    Combining clopidogrel and NSAIDs may increase blood loss and bleeding in the stomach and intestines.
•    Do not combine clopidogrel and other antiplatelet drugs or
the anticoagulant (blood thinner) warfarin unless you are
under your doctor’s direct supervision. This interaction may
prevent normal blood clotting and lead to severe bleeding
problems.

General Information
Clorazepate dipotassium is a benzodiazepine. Benzodiazepines directly affect the brain. They can relax you and make you more tranquil or sleepier, or they can slow nervous system transmissions in such a way as to act as an anticonvulsant. Many doctors prefer benzodiazepines to other drugs that can be used to similar effect because they tend to be safer, have fewer side effects, and usually work as well, if not better.
Cautions and Warnings
Do not take clorazepate if you are allergic or sensitive to any of its ingredients or to another benzodiazepine drug, including clonazepam.
Clorazepate can aggravate narrow-angle glaucoma, but you may take it if you have open-angle glaucoma and are receiving therapy for it.
Other conditions in which clorazepate should be avoided are: severe depression, severe lung disease, sleep apnea (intermittent cessation of breathing during sleep), liver disease, drunkenness, and kidney diseaap_, to inOn of these conditions, the qq)NSSvve effects of clorazepate may be enhanced or could be detrimental to your overall condition.

Clorazepate may be addictive. It should be used with caution in people with a history of drug dependence.
Drug withdrawal may develop if you stop taking it after as few as 4 weeks of regular use but is more likely after longer use. It may start with anxiety and progress to tingling in the hands or feet, sensitivity to bright light, sleep disturbances, cramps, tremors, muscle tension or twitching, poor concentration, flu-like symptoms, fatigue, appetite loss, sweating, and changes in mental state. Your dosage should always be reduced gradually to prevent drug withdrawal symptoms.
Possible Side Effects
Weakness and confusion may occur, especially in seniors and in those who are more sickly.
✓    Most common: mild drowsiness during the first few days of therapy.
✓    Less common: confusion, depression, lethargy, disorientation, headache, inactivity, slurred speech, stupor, dizziness, tremors, constipation, dry mouth, nausea, inability to control urination, sexual difficulties, irregular menstrual cycle, changes in heart rhythm, low blood pressure, fluid retention, blurred or double vision, itching, rash, hiccups, nervousness, inability to fall asleep, and occasional liver and kidney dysfunction. If you have any of these symptoms, stop taking the medicine and contact your doctor immediately.
✓    Rare: Rare side effects can affect your heart, stomach and intestines, urinary tract, blood, muscles and joints. Contact your doctor if you experience any side effects not listed above.
Drug Interactions
•    Clorazepate is a central-nervous-system depressant. Don’t mix it with alcohol, other sedatives, narcotics, barbiturates, monoamine oxidase inhibitor and other antidepressants, and antihistamines. Taking Clorazepate with these drugs may result in excessive depression, tiredness, sleepiness, breathing difficulties, or related symptoms.
•    Smoking may reduce clorazepate’s effectiveness by in-
creasing the rate at which it is broken down by the body.
•    Clorazepate’s effects may be prolonged when it is mixed with cimetidine, contraceptive drugs, disulfiram, fluoxetine, isoniazid, ketoconazole, metoprolol, probenecid, propoxyphene, propranolol, rifampin, or valproic acid. Theophylline may reduce clorazepate’s sedative effects.
•    If you take antacids, separate them from your clorazepate dose by at least 1 hour to prevent them from interfering with the absorption of clorazepate into the bloodstream.
•    Clorazepate may increase blood levels of digoxin and the chances of digoxin toxicity.
•    The effect of levodopa + carbidopa may be decreased if it is taken together with clorazepate.
•    Combining clorazepate with phenytoin may increase phenytoin blood concentrations and the chances of phenytoin toxicity.
Food Interactions
Clorazepate is best taken on an empty stomach, but it may be taken with food if it upsets your stomach.
Usual Dose
Immediate-Release
Adult and Child (age 9 and over): 15-60 mg daily. The average dose is 30 mg in divided quantities, but dosage must be adjusted to individual response for maximum effect. Maximum recommended daily dose is 90 mg. For treatment of anxiety, clorazepate may be taken as a single dose at bedtime.
Child (under age 9): not recommended.
Sustained-Release
Adult: The sustained-release form of clorazepate may be given as a single dose, either 11.25 or 22.5 mg, once every 24 hours. Sustained-release tablets are not recommended for the initial dosage.
Child: not recommended.
Overdosage
Symptoms of overdose are confusion, sleepiness, poor coordination, lack of response to pain such as a pin prick, loss of reflexes, shallow breathing, low blood pressure, and coma. The victim should be taken to a hospital emergency room. ALWAYS bring the prescription bottle or container.
Special Information
Clorazepate can cause tiredness, drowsiness, inability to concentrate, or similar symptoms. Be careful if you are driving, operating machinery, or performing other activities that require concentration.

Heart failure, high blood pressure, angina pain, and cardiomyopathy.
General Information
Carvedilol was the first beta blocker approved for heart failure. It is also the only beta blocker approved for severe heart failure.
Carvedilol blocks both the alpha- and beta-adrenergic portions of the central nervous system. This dual action reduces the amount of blood pumped with each heartbeat and also decreases the risk of tachycardia (very rapid heartbeat). Carvedilol’s beta-blocking effects begin within an hour of taking the first dose; maximum blood-pressure-lowering occurs after 1 or 2 weeks. The drug also causes blood vessels to dilate (widen), allowing the heart to pump blood more efficiently.
Cautions and Warnings
Do not take carvedilol if you are allergic or sensitive to any of its ingredients, or if you have AV block, sick sinus syndrome or severe bradycardia (slow heart rate) without the use of a pacemaker.
Carvedilol should not be taken 13y patients with bronchial disease, qQQkVaS thronic bronchitis, emphysema, or asthma.
Carvedilol therapy should not be stopped suddenly due to the risk of worsening the heart condition.
In studies, carvedilol caused mild and reversible liver injury in about 1 of every 100 people who took it. Those with severe liver disease should not take this medication. Call your doctor at once if you develop signs of liver damage (symptoms include severe itching, dark-colored urine, flu-like symptoms, appetite loss, and yellowing of the skin or whites of the eyes).
Check with your doctor about continuing carvedilol if you are to receive general anesthesia; heart function that is depressed by anesthetics can worsen if carvedilol is used at the same time.
Make sure your doctor knows if you have diabetes. Carvedilol can mask signs of low blood sugar and may increase the effects of insulin or oral antidiabetes drugs, making it more difficult to recover from the effects of low blood sugar.
Carvedilol can mask symptoms of an overactive thyroid gland. Abruptly stopping carvedilol can trigger an attack of hyperthyroidism.
Possible Side Effects
Most side effects are considered mild or moderate.
✓    Most common: dizziness, sleepiness or sleeplessness, diarrhea, abdominal pain, slow heartbeat, dizziness when rising from a sitting or lying position, swelling of the hands or feet, sore throat, breathing difficulties, tiredness, back pain, urinary infection, and viral infection.
✓    Less common: extra heartbeats; palpitations; blood-pressure changes; fainting; reduced blood supply to the arms and legs (symptoms include aches, cramps, pain, or tiredness on walking, or pain in the foot, thigh, hip, or buttocks); tingling in the hands or feet; reduced sensation; depression; nervousness; constipation; gas; liver irritation; cough; impotence and reduced sex drive in men; itching; rash; visual difficulties; ringing or buzzing in the ears; high blood cholesterol, sugar, or uric acid; anemia; weakness; hot flushes; leg cramps; dry mouth; not feeling well; sweating; and muscle ache.
✓    Rare: Rare side effects can affect the heart, mental status, the respiratory tract, the urinary tract, and the kidney. It can also cause hair loss, weight gain, high blood-triglyceride levels, low blood-platelet counts, and sugar in the urine. Contact your doctor if you experience any side effect not listed above.
Drug Interactions
•    Carvedilol increases the effects of insulin and oral antidiabetes drugs. People taking this combination must monitor their blood sugar levels regularly. Call your doctor if there is any change from your normal pattern.
•    Carvedilol increases the effects of verapamil, diltiazem, and similar calcium-channel blocking drugs.
•    Monoamine oxidase inhibitor antidepressants may increase the effects of carvedilol.
•    Carvedilol increases the blood-pressure-lowering effect of clonidine. People taking this combination may need less clonidine to control their pressure.
•    Carvedilol increases the amount of digoxin in the blood by about 15%. Your digoxin dosage may have to be adjusted.
•    Cimetidine increases the amount of carvedilol absorbed into the blood by about 30%, but the importance of this interaction is not clear.
•    Rifampin reduces the amount of carvedilol in the blood by about 70%. Dosage adjustment is necessary.
•    Do not consume alcohol (including medicines that contain alcohol) within 2 hours of taking carvedilol.
Food Interactions
Take carvedilol with food to reduce the risk of dizziness or fainting.
Usual Dose
Heart Failure
Adult: 3.125 mg twice a day for 2 weeks. Dose may be doubled every 2 weeks to the highest level tolerated. Maximum daily dosage is 25 mg twice a day in people weighing less than 187 lbs., and 50 mg twice a day in people who weigh more.
High Blood Pressure and Cardlomyopathy
Adult: 6.25 mg twice a day to start, increased to 25 mg twice a day if needed.
Senior: Seniors may require smaller doses than younger adults. Child (under age 18): not recommended.
Overdosage

People taking carvedilol may become dizzy or faint when rising quickly from a sitting or lying position. If this happens to you, sit or lie down until you feel better. Carvedilol can also cause drowsiness, lightheadedness, or blurred vision. Be careful when driving or doing any task that requires concentration.
Contact lens wearers are more likely to experience dry eyes with carvedilol.
Swallow extended-release tablets whole; do not crush or break them.

General Information
Brimonidine tartrate stimulates alpha-2 receptors in the eye and lowers pressure there. The maximum effect occurs 2 hours after the drops are administered. Brimonidine reduces the amount of aqueous humor (liquid) produced inside the eye and increases the rate at which fluid flows out of the eyeball. That portion of brimonidine that finds its way into the bloodstream is broken down by the liver.
Cautions and Warnings
Do not use brimonidine if you are allergic or sensitive to any of its ingredients.
People with kidney or liver disease should use this drug with caution. People with cardiovascular disease should exercise caution with this medication because it can affect blood pressure. It should be used with caution in people with depression, cerebral or coronary insufficiency, and Raynaud’s disease. Brimonidine may cause fatigue, drowsiness, and dizziness or fainting when rising from a sitting or lying position.
Brimonidine’s effectiveness may decrease over time. Your doctor should check your eye pressure periodically to make sure the drug is still working.
Possible Side Effects
✓    Most common: dry mouth; redness, burning, and stinging of the eye: headache; blurred vision; sensation of something in the eye; drowsiness; and eye allergy and itching.

General Information
Chemical cousins of aspirin, these bowel anti-inflammatory drugs (5-ASA type) are used to treat symptoms of bowel inflammation. No one knows exactly how they work, but they are believed to have a local effect on the bowel. The tablet forms are made to delay drug release until they reach the colon. Little of the drug is absorbed into the blood; 70-90% remains in the colon.
Cautions and Warnings
Do not take bowel anti-inflammatories if you are allergic or sensitive to any of their ingredients or to aspirin. Although people who are sensitive or allergic to sulfasalazine have generally been able to tolerate mesalamine—which is an active agent in sulfasalazinethey should be cautious.
Bowel anti-inflammatories may worsev\ 4zo%Vis or cause cramping sudden abiboYrimall pain, bloody diarrhea, fever, headache, or rash. Stop taking the drug at once and call your doctor if any of these symptoms develop.
Some people taking mesalamine have developed kidney problems. People who have or have had kidney disease should be cautious about using these drugs. All people taking mesalamine should have kidney function tests before and during drug therapy.
Possible Side Effects
Bowel anti-inflammatories are generally well tolerated. Tablets and capsules have the most side effects, suppositories the least.

Capsules: 1000 mg 4 times a day for up to 8 weeks.
Suppositories: one 500-mg suppository twice a day for 3-6 weeks. Retain the suppository for 1-3 hours for maximum benefit.
Rectal Suspension: 1 bottle of suspension taken as an enema at bedtime every night for 3-6 weeks. The enema liquid should be retained for about 8 hours.
Olsalazine
k4kft, MO mg a day in 2 divided doses.
Overdosage
Symptoms are likely to include: ringing or buzzing in the ears, fainting or dizziness, headache, lethargy, confusion, drowsiness, sweating, rapid breathing, vomiting, and diarrhea. In case of overdose, call your local poison control center or hospital emergency room. You may be told to induce vomiting with ipecac syrup—available at any pharmacy—before taking the victim to the emergency room. If you seek treatment, ALWAYS bring the prescription bottle or container.
Special Information
The tablets and capsules must be swallowed whole. Call your doctor if they are visible in your stool. When using suppositories, handle them as little as possible to prevent melting.
Call your doctor if you develop chest pain, breathing or urinary difficulties, fever, unusual bleeding or bruising, worsening of colitis, or any bothersome or persistent side effects.
If you forget to administer a dose, do so as soon as you remember. If you take a tablet or capsule and it is within 4 hours of your next dose, skip the dose you forgot and continue with your regular schedule. If you take the suppositories or rectal solution and you do not remember until it is almost time for the next dose, skip the one you forgot and continue with your regular schedule. Do not take a double dose.
Special Populations
Pregnancy/Breast-feeding: Bowel anti-inflammatories can pass into the fetal circulation. When your doctor considers these drugs crucial, their potential benefits must be carefully weighed against their risks.
Small amounts of these drugs can pass into breast milk. Nursing mothers who must take these drugs should consider using infant formula.
Seniors: Seniors may use these drugs without special restriction.

A-Z Principal Drugs (lithium succinate - menthol)

lithium succinate Lithium succinate appears to have sonic antifungal and anti-inflammatory properties, and is used as an 8% ointment for seborrhoeic dermatitis. (Ftalith).
lodoxamide A mast cell stabilizer similar to sodium cromoglycate. Used as eye drops (0.1%) in allergic conjunctivitis. (Alomide).
Dose: in acute diarrhoea, 4 mg initially, followed by 2 nig as required, up to a maximum of 16 ing daily. In chronic diarrhoea, 4-8 nig daily, but care is necessary in the elderly to avoid faecal impaction. Loperamide is not suitable for children under 4 years of age, nor in patients with liver disease, as it may cause undesirable sedation. (Iniodium).
loprazolam A benzodiazepine hypnotic used mainly in the short-term treatment of insomnia and nocturnal arousal. Dose: 1-2 nig at bedtime. Side-effects include drowsiness, dizziness, dry mouth and headache. See page 152 and Table 22.
loratadine An antihistamine with the general action of that group of drugs, but with reduced sedative side-effects.
Dose: 10 nig dailv. (Clarityn). See page I 10 and Table 2.

lofepramine An antidepressant of the irnipramine group, with similar actions and uses, but reduced sedative and anticholinergic side-effects.
Dose: 140-210 nig daily. (Gamanil). See page 128 and Table 11.
lofexidine A narcotic antagonist. It has a selective blocking action on brain nor-adrenaline, and is used for the rapid relief of opioid withdrawal symptoms associated with central sympathetic activity.
Dose: 200 pg twice a day, slowly increased as required over 7-10 days, before withdrawal over 2-4 days. Care is necessary in cardiac insufficiency and bradycardia. (Britl.olex).
lomotil A preparation of diphenoxylate with atropine, for the rapid control of diarrhoea. Dose: 2 tablets 6-hourly.
lomustine A slow-acting cytotoxic agent used in Hodgkin’s disease and solid tumours.
Dose: 130 ing/ni’body surface at intervals of (> 8 weeks. Side-effects, include anorexia. nausea, liver damage and niyelodeprm ion. Dosage should not be repeated until white cell and platelet counts have returned to an acceptable level. Reduced doses are given
when lomustine forms part of a multi-drug dosage scheme. (CCNU). See page 122 and Table 8.
loperamide A synthetic inhibitor of peristalsis.
lorazepam A short-acting anxiolytic/ hypnotic similar to diazepam, but less likely to cause next-day drowsiness. Dose: 1-4 mgdaily. It is also given in similar oral closes or by slow i.v. injection in doses of 50 pglkg for preoperative sedation and anuiesia. Occasionally used i.v. in status epilepticus in doses of 4 nig, but apnoea and hypotension are side-effects that may require resuscitation. fAtivan). See page 152 and Table 22.
lormetazepam A short-acting benzodiazepine hypnotic. It is useful in the treatment of insomnia in the elderly, but is less suitable for insomnia associated with early awakening.

losartanV An angiotensin 11 receptor antagonist used in the treatment of hypertension.
Dose: 50ing daily The use of potassium-sparing diuretics should be avoided with losartan. It has the advantage of not causing the persistent dry cough associated with ACE inhibitors. (Cozaar). See page 148 and Table 21.
low molecular weight heparins See heparin.
loxapine Antipsychotic agent with the actions and uses of chlorpromazine. Dose: in acute and chronic psychoses, 25-50 mg daily, slowly increased as required. Maintenance doses range from 20-100111g daily. Side-effects are those of other anti-psychotic agents. but loxapine may cause nausea, vomiting and weight changes. f .oxapac). magnesium hydroxide A mild antacid laxative, usually given in aqueous suspension as Cream of Magnesia, although tablet forms are also available. Cream of Magnesia is a useful antidote in mineral acid poisoning.

Lugol’s solution An aqueous solution of iodine 5% and potassium iodide 10%. Used in the preoperative treatment of t hyrotoxicosis.
Dose: 0.3-1 ml.
lymecycline A soluble complex of tetracycline and lysine. It has the action and uses and side-effects of tetracycline, but is absorbed more readily.
Dose: 800 mg daily. (Tetralysal).
typressin An analogue of vasopressin used to control the polyuria of pituitary diabetes insipidus.
Dose: 2.5-10 units several times a day by nasal spray. Side-effects include nausea and abdominal pain. I.ypressin has some vasoconstrictor properties, and desmopressin is sometimes prellcrred. (Syntopressin).
lysuride (lisuride) A bromocriptine-like drug for the treatment of parkinsonism. II acts by stimulating any surviving dopamine receptors in the brain.
Dose: 200 pg at night with food,
irk ceased at weekly intervals according to response up to a maximum of 5 mg daily. Side-effects include nausea, dizziness and initial hypotensive reactions which may affect driving ability. (Revanil). See
page 160 and Table 26.
magnesium sulphate Epsom salts. A powerful saline aperient, producing loose stools by preventing the reabsorption of water.
Dose: 5- 15 g before breakfast. Used externally for the treatment of boils and carbuncles as a paste with glycerin. A marked loss of plasma magnesium may occur after severe diarrhoea or drug-induced diuresis, and may require the i.v. infection of magnesium sulphate in doses based on the degree of hypoinagnesacmia. It has also been given i.v. in a dose of 8 mmol in the emergency treatment of severe;U’rhythinias associated with hypokalaemia.
magnesium trisilicate A white insoluble powder, with mild but prolonged antacid effects. It was formerly widely used in the symptomatic treatment of peptic ulcer;
now used chiefly for dyspepsia.
Dose: 0.3-2 g.
malathion An organophosphorus insecticide. Used as a lotion 0.5% for lice and scabies as alternative to lindane or carbaryl.
mannitol A sugar that is not metabolized, and is used mainly as an osmotic diuretic. Dose: (after a test dose of 200 mg/kg) 50-200 g by slow i.v. infusion over 24 hours. Mannitol has also been used by i.v. infusion as a short-term ocular hypotensive agent in the treatment of glaucoma. It is also useful in cerebral oedema, given by rapid i.v. injection in a dose of I g/kg as a 2044, solution.
macrolides A group of antibiotics that differ chemically from the penicillins, yet have a similar pattern of activity. They are active orally and are useful in the treatment of penicillin-sensitive patients. Erythromycin is the most widely used member of the group, with clarithromycin and azithromycin as more recent introductions.
magnesium carbonate A white, insoluble powder with antacid and laxative properties.
Dose: 0J-4 g daily.
maprotiline A sedative antidepressant with a general action similar to that of the tricyclic drugs represented by amitriptyline. Dose: 25-150 mg daily. If given at night as a single dose, the sedative action may reduce the need for other drugs. It has milder anticholinergic side-effects than some related compounds, although skin rash is more common. (I udionlil). See page 128 and Table 11.
mebendazole An anthelmintic effective against most intestinal worms.

Dose: 100 mg once for threadworm, and 100 mg twice daily for 2 days against other infestations. Generally well tolerated, but it should not be given to children under 2 years of age. (Verniox).
I Men
in oedematous states. A potassium supplement may be required. Care is necessary in renal and hepatic deficiency.
(KiYcaron). See page 148 and Table 21.
mebeverine An antispasmodic agent which, unlike the anticholinergic drugs, appears to have a direct action on the intestinal smooth muscle. It is useful in the treatment of gastrointestinal spasm and in the irritable bowel syndrome. Dose: .100 mg daily, before food. As with other antispasmodics, mebeverine should not be used in paralytic ileus. (Colofac).
medroxyprogesterone A synthetic progestogen.
Dose: in endometriosis 30 mg daily for 90 days; in dysfunctional uterine bleeding and secondary amenorrhoea: 2.3-10 mg daily for 5-10 days, starling on l6th-2 Ist day of cycle and repeated for 2-3 cycles. Large doses of 400 mg-1.5 g daily are given in breast, endometrial, prostate and other hormone-dependent cancers, or 250mg– I g weekly by deep i.m. inJection. (Farlutal; proves). Depot-proves is a long-acting product used by i.m. injection is a contraceptive. but only after fit][ counselling.
megestrol An orally active progestogen. It is used in oestrogen-dependent breast cancer, and acts by suppressing the uptake of oestrogens by the cancer cells.
Dose: 160 mg daily. Nausea and fluid retention with weight gain are occasional side-effects. (Megace). See page 122.
meloxicarn A recently introduced non-steroidal anti-inflammatory drug (NSAID) indicated in the short-term treatment of acute osteo-arthritis and the longer-term treatment of rheumatoid conditions. Dose: 7.5-15 mg once daily with food; half doses for the elderly. Suppositories of 15 mg are also available. The side-effects are basically those of the NSAI Ds in general. Meloxicam has a more selective action on cyclo-oxygenase, the enzyme involved in the biosynthesis of prostaglandins, and is less likely to cause gastrointestinal disturbance, but it has no cytoprotective action, and is not suitable for patients with peptic ulcer. (Niobic). See page 165 and Table 29.

mefenamic acid A non-steroidal anti-inflammatory analgesic agent used to relieve moderate pain in arthritic and rheumatoid conditions, and other states requiring mild analgesic therapy such as dysmenorrhoea. Dose: 1.5 g daily after food. Side-effects are drowsiness an(] haemolytic anaemia. Diarrhoea is an indication that the drug should be withdrawn. (Ponstan). See page 165 and Table 29.
rnefloquine A drug for the prophylaxis and treatment ofchloroquine-resistant malaria. Dose: lot- short -term prophylaxis 250 mg weekly, starting 1 week before exposure and for 4 weeks after return. Doses for treatment require specialist advice. Side-effects include gastrointestinal disturbances, dizziness and weakness. It is contraindicated in patients with a history of neu ro- psych iatric disturbance, and is not suitable for use in severe renal or hepatic impairment. (Lirium). See halofantrine.
mefruside A diuretic useful in the treatment of hypertension and oedema. Dose: 25-50 mg daily in the morning, according to need and response; 25-100 mg
melphalan An alkylating agent of the mustine type. Used mainly in myelomas, lymphomas and some solid tumours. Dose: 150-300 gg/kg daily for 4-6 days, repeated after 1-2 months. In myeloma it is also given by regional perfusion. The injection solution is highly irritant and contact should be avoided. Side-effects include myelo-depression, nausea, rash and pruritus. (Alkeran). See page 122 and ‘rabic 8.
menadiol A water-soluble form of vitamin K. Dose: 10 ing daily. (Synkavit) Phytorneii,dionc i.% now preferred.
menotrophin Human menopausal gonadotrophin containing follicle-stimulating hormone and luteinizing hormone. It is used in the treatment of anovulatory sterility. The dose depends on individual hormone assays and response. The use of the drug has resulted in multiple births. It is also given to males to stimulate spermatogenesis. (Humegon; Normegon).
menthol Colourless crystals obtained from oil of peppermint. Used as spray or drops for nasopharyngeal inflammation.

Allergens:House-dust mite and insect pests

Because house-dust mites are a major source of allergic reactions they have been studied intensively, and various ways of killing them devised. But simply killing the mites is not enough. Their allergens will remain, and continue to cause allergic reactions for years. The allergens have to be either removed or inactivated — that is, changed chemically so that they are no longer recognised by the immune system.
Tackling dust mite is easier if you know certain key facts:
1 Dust mites prefer humid conditions. They do not drink, but absorb water from the air. When the relative humidity falls below 50%, the mites gradually dry out and are killed.
2 Mites feed on our skin scales, but only if they have been broken down first by moulds. High humidity (70-90% relative humidity) is a particular problem, for anyone with an allergy to house-dust mites, because it favours the mould that suits dust mites best.
3 Dust mites live inside mattresses, pillows, upholstery, cushions and soft toys. The allergens are blasted out when you settle into an armchair, get into bed, or turn over in the night.
This is when you inhale the biggest dose of allergen, or get the maximum dose to your skin. Carpets also contain dust mites, but the numbers are generally lower.
4 Dust-mite allergen is relatively heavy, compared to cat or mould allergens for example. Little of it floats around in the air, and the most significant exposure is inhaling it close to the source — from a pillow, mattress or teddy bear. This is why air filters are of little value for anyone with dust-mite allergy.
5 Dust mites are everywhere, and are carried around in clothing. Even if you could eliminate all the mites from your house, new ones would soon appear. A new mattress will usually be colonised by dust mites within four months.
The size of the allergen particles
The droppings of the dust mite, not the mites themselves, are the main cause of symptoms. The droppings are 4-20 microns in size, but they can crumble into fragments of 1-3 microns, and the tiniest bits are only 0.5 microns across. The pores of mite-proof covers (which really means mite-allergen-proof) should be less than one micron across, and preferably less than 0.5 microns. Dust masks (see p. 109) should also filter out particles of this size to be effective. The mites themselves are much bigger, 200-300 microns long, (A micron is one thousandth of a millimetre.)
The basics of mite warfare
• A temperature just above boiling point kills dust mites and inactivates Der pl, which is the troublesome allergen for most asthmatics. However Der p2, the other mite allergen, is not affected by heat. (Note that the carpet treatments advertised as ’steam cleaning’ generally just use hot soapy water, not steam. Because they leave the carpet very damp, they can increase the numbers of dust mites.)
• Washing with detergent at 55′C (130′1 or above (i.e. a 60′C wash cycle) kills mites and removes the allergen.
• Cooler washes will not kill mites, but will remove the allergen. This can be useful if the mites have already been killed by some other means. Regular cool washes of clothing or sheets will also remove human skin scales, reducing the mites’ food supply. (This is beneficial if you have eczema, because flaking skin adds to the problem by giving dust mites even more to eat.)
• You can buy mite-killing substances (see p. 255) to add to cooler washes, so that the mites are killed - the chemicals are rinsed out at the end of the wash, so are pretty safe.
• Dry-cleaning kills mites and it removes some of the allergen, but the amount removed is variable (20-70%).
• Freezing for more than six hours kills mites. Three hours’ strong direct sunlight in dry air will kill mites living in rugs and blankets. Neither treatment removes allergen.
• Mites hang on to the carpet fibres when the vacuum cleaner passes overhead, and about 65% of them remain afterwards. An ordinary vacuum cleaner sprays mite allergen into the air as it goes. The amount in the air - and therefore available to be inhaled - is three times higher after vacuuming.
Combating mites
Bear in mind that mites are the enemy - not dust itself. A house may be thick with dust but, because the windows are open a lot and the air is dry, it will have few mites. Another house may look perfectly clean, but be seething with mites because it is thoroughly draught-proofed, warm and slightly humid. The mites will be thriving in the carpets, beds and upholstery. Vacuuming and dusting every day, if done with an ordinary type of vacuum cleaner and a dry duster, will stir up the allergens and ensure that the air is full of them. So a person with mite allergy would feel far worse in the apparently very clean house than in the dusty one.
One crucial aspect of a mite-reduction programme is making the air drier - see p. 119 - so that mites no longer flourish.
Most of the other measures - described below - will involve stirring up dust-mite allergens, so the allergic person should not do the work, nor be in the house (see p. 109).
Too dry or too moist?
It is a well-established fact that the air in most modern houses is too humid, encouraging dust mites and moulds. Yet many people fit humidifiers because they believe that the air is ‘too dry’ and that this irritates the nose. Some very good scientific studies have shown it is indoor pollutants plus overheating that is the problem here, not dryness - even very dry air is not irritating as long as it is clean. Should your nose feel dry and ticklish, try to reduce indoor pollution (see pp. 128-9).
It is true that during an asthma attack, dry air does make matters worse, and very moist air helps. Inhaling steam from a bowl of hot water can be used to ease the attack.
The bed
Begin with the bed because this is the main exposure zone. A Danish study showed that just fitting mite-allergen-proof covers to the mattresses and pillows of dust-allergic children worked well. After a year the children had much less asthma at night, used half as much inhaled steroid, and gave better peak-flow readings.
The best approach is to buy a new mattress and new pillows before putting anti-mite coverings on them. These covers keep skin scales and mites out, which should prevent a new mattress or pillow becoming recolonised. The modern covers have tiny pores which allow perspiration to evaporate - this makes them comfortable to sleep on.
These pores are small enough (see box on p. 114) to keep any mite allergens inside, so they will also work with an old mattress, keeping the existing allergen inside. But the mites themselves will also thrive inside (there’s enough old skin there to keep them in business for years) and there is always the risk that, if a small tear develops, the stores of allergen in the mattress will come pouring out again. So start with a new mattress if you can.
Another possible option is to arrange for a contractor to heat-treat the bed, the mattress and all bedding. This is a new specialist treatment (see p. 255), where the bed is enclosed in a plastic tent and heated to very high temperatures. It is designed to kill all mites, even those right inside the mattress, and inactivate the allergen. (The contractor can also do your living room suite.)
If the covers are for a small child, check with the manufacturer that they pose no threat of suffocation. Mattresses and bedding with built-in covers may be safer.
Buy a new duvet (or wash or dry-clean your existing one) and put an anti-mite cover on it. Alternatively, buy a duvet and pillow that can be washed at 60′C (130′F), and wash them once a month. You must have the use of a tumble dryer, because mites will flourish if bedding is not completely dry.
An upholstered bed base will have its own (much smaller) population of mites. Buy a simple wooden or metal bed frame if possible. Or you could enclose the upholstered base in a mite-proof cover, or in plastic sheeting completely sealed with heavy-duty tape.
Wash all sheets and blankets at 60°C or more, or have blankets dry-cleaned – or buy new ones. From now on, wash sheets once a week and blankets once every two weeks.
Get rid of any other bedding such as patchwork quilts or fleecy underblankets. Alternatively, you can wash or dry-clean them regularly.
Electric blankets can be cleared of mites by washing them, and are very useful in keeping the bed free from moisture. This prevents mites from setting up home in the outer surface of your new mite-proof covers, so that you don’t need to wash the covers, sheets and underblankets so frequently. Leave the electric blanket on at a high setting, with the bed made, for at least twelve hours (check that there is no fire risk first). Note that some mite-proof covers might be damaged by this procedure – check with the manufacturer. If you have not yet purchased mite-proof covers, there are some made from Egyptian cotton which can tolerate this level of heat without damage (see p. 255).
Children’s beds and toys
Where children share a room, all the beds and bedding should be dealt with. Even then, an asthmatic child should never sleep in the lower half of a bunk bed, because mite allergens will shower down from the bed above.
Ali soft toys should spend at least six hours in the freezer once a week, to kill the mites. The first time, wash the toys
immediately afterwards to remove any existing allergen and dry thoroughly in a tumble dryer.
A hot wash, or the freezing/washing treatment, should also be used for ‘comfort blankets’, dressing-up clothes, dolls’ clothes and any other fabric items.
Sheepskins, sometimes used for babies’ cots, especial,. New Zealand and Australia, contain huge amounts of dust-mite allergen. It is advisable to discard these.
The next steps
Clothing is often full of mites, especially sweaters, coats and woollen trousers. Dry-clean all such items, or wash using a m –e-killing wash, then store them in a well-heated place so that they are always very dry.
Dandruff consists of skin flakes, and may help to feed mites Using an anti-dandruff shampoo may help. Semen also gives mites nourishment.
From now on, be careful about exposing your airways to dust. Get someone else to empty the vacuum cleaner bag – and they should, of course, do it outdoors. If you are stripping wallpaper, wash it down first to remove dust. Moving house, going into the attic, spring cleaning, turning out cupboards or moving furniture should all be avoided – unless you have a good mask on.
Do not use fan-heaters or convector heaters which churn up mite allergens from the carpet. Seal off any hot-air ducts from centralised heating systems, as these blow mite allergens around the room.
If possible, invest in a vacuum cleaner that keeps in all the allergens, or vents them outside, rather than spraying them out into the air. Make sure that the vacuum cleaner you buy really
What about sprays?
Chemicals that kill mites (known collectively as acaricides) are sometimes useful but have various limitations. They do not penetrate inside upholstered furniture, cushions or mattresses, so make little difference to the total population of dust mites. Even on carpets, sprays won’t reach most of the mites unless you rub the spray in really hard. (And ‘anti-mite’ carpet shampoos are completely ineffective.)
The safest chemical is benzyl benzoate — so safe that it is used directly on the skin for treating scabies infections. It can cause skin irritation at these doses, but rarely does so at the concentrations used in anti-mite sprays.
However, the idea of constant spraying, over a period of months or years, is worrying. Doctors generally advise against spraying bedding, and carpets or furniture where babies or small children play, to avoid close and prolonged contact with the spray residue.
Even more alarming are sprays containing a pyrethroid (pyrethrum) compound. The latter is derived from a plant and is therefore sold as ‘natural’, but pyrethroids are potentially toxic with prolonged exposure, and they quite often provoke allergic reactions too. They should definitely be avoided.
Sprays that inactivate allergen (rather than killing mites) sometimes have their uses. There are two kinds and both should work against a variety of allergens, not just dust mite. Polysaccharide sprays stick the allergen particles together, so that they don’t float about and get inhaled. Tannic acid sprays change the allergen chemically, making it non-allergenic. Because tannic acid is found in tea it is assumed to be harmless, but the sprays available vary a lot and often contain many impurities, so it is hard to be sure about their long-term safety. Don’t use these sprays on bedding.
Carpets and bedding covers with built-in pesticide are also on sale, but are probably best avoided.
does its job well – a lot of machines now claim to be ‘allergy’ vacuum cleaners but they are not all equally good. Very few have been adequately tested (see p. 255). Alternatively, cover the bed with a clean sheet and open the windows whenever you vacuum, leaving them open for half an hour afterwards. After closing the windows, allow the dust to settle for another half hour, then carefully remove the dust-cover from the bed.
For dusting, use a damp cloth and add a few drops of eucalyptus oil which deters mites. Alternatively, use a special anti-mite duster with an electrostatic charge that holds the dust.
Above all, keep the moisture levels in your house down. Ultimately, this is the key to eliminating dust mite. Look at p. 119 and check you are doing everything possible.
The bedroom in particular should be kept dry. Air your bedroom whenever it’s dry and sunny. Remove pot plants and fish tanks. Don’t dry clothes in the room and don’t shampoo the carpet. Avoid using Calor gas heaters, as these produce a lot of moisture. If your bedroom has an en suite shower, fit a powerful extractor fan, or open a window wide during and after showers – or just stop using this shower. En suite basins may also generate moist air.
Do you need to do more?
Give it some time before deciding if you have done enough. In one study, it took eight months for the full benefits of an anti-mite campaign to be seen.
If you are still not as much improved as you hoped, then you could try a more drastic mite-elimination programme.
Thoroughly clean the bedroom, getting rid of any dust along skirting boards or picture rails, on top of wardrobes or behind furniture. Remove anything stored under the bed, so that vacuuming is easier in future. During this cleaning operation, completely cover the bed.
Get rid of the bathroom carpet, if you have one. In the bedroom, either remove the carpet or buy a special anti-mite steam cleaner that kills mites in the carpet and inactivates the allergen. To work properly, the device must produce steam at a temperature above boiling point, by means of high pressure. Make sure you are buying the right kind of device.
If you take out the carpet, you will need to mop the floor, with a wet or oiled mop, several times a week, as the dust will quickly build up, and is easily made airborne from an uncarpeted floor. One of the advantages of carpet is that it ‘holds’ dust at floor level.
Wash the curtains, or dry-clean them, or replace them with blinds of a kind that can be easily wet-dusted. If you have bought an anti-mite steam cleaner for the carpet, use this on the curtains every 2-3 weeks.
Remove dirty clothes from the bedroom, clean out drawers and shelves, and dry them thoroughly. Only store freshly laundered clothes in the room.
Remove all upholstered items from the bedroom, such as padded headboards, cushions, armchairs, or stools with padded seats. Draught excluders, fabric lampshades and anything covered in velvet should also go.
A different approach
If you are even more allergic to housework than you are to dust mites, consider buying a really powerful dehumidifier, designed for killing mites. This makes the air too dry to breathe (its relative humidity or RH goes down to 25%), so you leave it on in the bedroom during the day, with the bedroom door closed. You must eliminate all sources of moisture that will counteract the dehumidifier, and have fairly tight seals around your windows and doors for it to work. In the evening, turn the dehumidifier off and leave the bedroom door open for an hour or so before going to bed.
Of course, all the allergen which was already there in the bed, carpet, curtains, clothes and soft toys will still be present. You need to either eliminate or inactivate this allergen using the methods described above. But once you have got your daily dehumidifier routine going, you do not need to rewash everything regularly because mites will be a thing of the past, so no new stocks of allergen will be produced.
The rest of the house
A completely mite-free house is hard to achieve, but if you are determined, you can come close. Everything so far described for the bedroom, such as reducing moisture in the air, and dealing with carpets and curtains, is applicable to the rest of the house.
The exception – and the toughest nut to crack – is the upholstered furniture in the living room. Fixed upholstery (i.e. everything other than removable cushions) is a safe haven for mites that is especially hard to deal with.
One option is to give all such furniture a specialist heat-treatment, if this is available locally (see p. 115). The mites inside will be killed, and the allergen inactivated. If you drastically reduce moisture levels at the same time – with a powerful dehumidifier used at night, perhaps – you should avoid serious reinfestation.
Alternatively, you could replace all your existing upholstered furniture with leather-covered or vinyl-covered furniture. Both are impenetrable to mites. Furniture made of wood or bamboo with loose cushions and no fixed upholstery can also work. Fit the cushions with tailor-made mite-proof covers (hard to get, but ask around) when new. Then put the ordinary covers on top, and wash these regularly.

Allergy and Your Immune System
`The summer used to be such a miserable time for me because I’m allergic to grass pollen. For most of

my life I have had dreadful hayfever, and my asthma would get worse during the summer as well.

Antihistamines knocked me for six, and although there were nose drops that helped a little, they

certainly did not resolve the problem completely. Exam time was always a nightmare when I was a student

- then, as now, it coincided exactly with the pollen season.’
‘Getting a job in Chicago was a turning point in my health. My colleagues were amazed to see me

snuffling through the summer and just accepting that nothing could be done to improve matters. The

whole approach to treating allergies is different there. Eventually someone marched me off to see her

allergist, who said that I should have “allergy shots” and that my health insurance would cover it. The

process was very time-consuming at first, and it took a while to work, but the change is remarkable.

I’ve never regretted having the treatment. Summer is a time I can actually enjoy now.’
Not everyone responds this well to immunotherapy, but for those allergy sufferers who do benefit, this

is an excellent treatment. It tackles allergies right at their source, by teaching the immune system to

react differently to the allergen.
Also known as Specific Immunotherapy (SIT), Incremental Immunotherapy (11T) or simply as

hyposensitisation, this form of treatment was devised by two English medical researchers, Leonard Noon

and John Freeman, who reported their successes with hayfever patients in 1911. Ironically, their

treatment is now less readily available in Britain than in any other industrialised nation. Only a

small minority of British allergy patients receive immunotherapy. The cause of this strange situation

is a ruling made in 1986 by the Committee on the Safety of Medicines (CSM). This states that

immunotherapy must only be given where there is resuscitation equipment available, and that all

patients must wait for an hour after each injection, in case of
side effects. In addition, immunotherapy cannot be used for severe asthma.
The requirement for resuscitation equipment rules out most GP surgeries, and this effectively puts

immunotherapy beyond the reach of many allergic individuals in Britain, owing to the extreme shortage

of allergists and hospital allergy clinics (see p. 89). (In the past, the lack of allergy specialists

meant that most immunotherapy in Britain was given by GPs.)
The CSM ruling was triggered by a number of deaths due to immunotherapy: there were eleven fatalities

between 1980 and 1986, with five of these in the eighteen months just before the report. But almost all

these deaths were due to very basic errors in the way the injections were given – tragic as the deaths

were, the official response to them was inappropriate. Fatal reactions to immunotherapy can be avoided

with close attention to ordinary safeguards (see p. 166-7).
Allergen immunotherapy is still freely available elsewhere in the world, and is regarded as a key part

of allergy treatment. Britain is now out of step with all other developed countries, and most doctors

feel that British restrictions are far too strict.
There are hopes that this situation may change within the next few years, and that more allergy

sufferers may be able to take advantage of this valuable treatment. This could be achieved, in part, by

investing more National Health Service money in allergy clinics and allergy specialists. In addition,

there should be a relaxation of the regulations, so that properly trained GPs can give immunotherapy to

patients who are not at high risk of a fatal reaction. For people whose lives are affected by

allergies, the reintroduction of this treatment (with appropriate safeguards) would be a huge boon.
The uses of immunotherapy
Immunotherapy is mainly used for airborne allergens such as pollen, house-dust mite and mould spores.

Allergies to animals can also be treated with immunotherapy, but the treatment cannot work miracles –

if a cat-allergic person decides to keep the cat, the high dose of allergen inhaled every day limits

the impact of immunotherapy treatment.
People with straightforward allergic reactions affecting the nose and eyes (allergic rhinitis and

conjunctivitis) respond well to immunotherapy. In patients with hayfever, for example, the success rate

(patients showing some degree of improvement) is about 80-90%. When nasal allergies are complicated by

chronic sinusitis or nasal polyps, the chance of success is a little lower.
Some studies of the long-term effects of immunotherapy suggest that, if it is given to children with

hayfever or perennial rhinitis, those children are less likely to develop asthma.
The benefits of using immunotherapy to treat established asthma are less certain. Asthma is a more

complex disease than hayfever, and allergies are only one factor among many (see p. 36), which may

limit the impact that immunotherapy can make. Experience suggests that immunotherapy can be a great

help for an asthmatic with a strong allergic reaction to a single airborne allergen, such as grass

pollen or house-dust mite, but not for other asthmatics. Asthmatics with aspirin sensitivity or chronic

sinusitis are unlikely to benefit.
The value of immunotherapy to children with asthma is a subject of great debate among doctors in the

United States. Some studies suggest that it is of little real benefit, while others are more positive.

One interesting study, that followed asthmatic children for 15 years or more, found that if they were

given a full five-year course of immunotherapy when young, they tended to have fewer asthma symptoms

and need less medication in their late teens and early twenties.
Chronic urticaria (nettle rash) is occasionally due to airborne allergens, in which case immunotherapy

may help. However, immunotherapy is not recommended for atopic eczema. When people with both eczema and

rhinitis try immunotherapy for their nasal allergies, some find that their eczema gets worse.
Insect-sting allergy is a prime candidate for immunotherapy (see pp. 167-8) but food allergy is a

different matter, and is not treated with immunotherapy at present (see p. 168).
Who can get immunotherapy?
As a result of the CSM ruling (see p. 164) remarkably few allergy sufferers in Britain receive

immunotherapy.
Those with insect-sting allergy, who have suffered anaphylaxis (see p. 58), are the most likely to be

offered this treatment. However, even with this frightening and life-threatening problem, which can be

treated with almost 100% success by immunotherapy (see p. 167-8), such treatment is not automatically

available.
A few people with severe hayfever that does not respond well to drug treatment may also be given

immunotherapy. It is worth asking your doctor about such treatment if you feel you would benefit.
How immunotherapy works
Immunotherapy consists of a series of small injections, just under the skin. The liquid that is

injected contains an extract of the offending allergen, for example house-dust mite. The injections are

given at regular intervals, usually once a week, although other schedules are possible (see p. 167-8).
At the outset, a very dilute version of the allergen extract is used, way below the threshold for an

allergic reaction. People who seem highly sensitive, on the basis of their skin tests, start on an

extract that is even more dilute.
For the next injection, a slightly higher concentration of the allergen extract is used, and the

concentration goes on increasing with each successive injection. The idea is to habituate the immune

system to the offending allergen, by very gradually raising the dose. Eventually, when the dose reaches

a level which generally gives beneficial effects, no further increases are made.
If an allergy sufferer reacts badly to immunotherapy injections (see p. 166) on several successive

occasions, the dose may be levelled off before the ideal maximum dose is reached. However, a good

allergist will persist for some time in trying to increase the dose because stopping at a lower level

often results in the treatment being ineffective.
The first stage of immunotherapy, when the concentration of allergen is being increased week by week,

is referred to as the build-up stage. The second stage, when the dose is being kept at the same level,

is called maintenance therapy, and the dose used is the maintenance dose.
There is sometimes an obvious improvement by the time the build-up stage is complete, but not always.

The benefits of the treatment generally appear within six months of reaching the maintenance dose, but

some people have to wait a year or even two before things improve. As the immunotherapy begins to take

effect, symptoms decline and there is often less need for drugs.
A great deal of research effort has gone into finding out what lies behind these changes – in other

words, what is actually happening to the immune system when immunotherapy is effective. The answer is

that a surprising number of different changes may occur and no two allergy sufferers react to

immunotherapy in quite the same way. Frequently there is a shift in the kinds of antibodies the body

produces against the offending allergen. Levels of IgG antibodies (which help to block the allergic

reaction) go up, while levels of the allergy antibody, IgE, tend to stabilise and eventually go down.

The numbers of mast cells (see box on p. 12) may also decline, and they can become less responsive to

the allergen. The balance of power between Th1 cells and Th2 cells may also shift, with the pro-allergy

Th2 cells (see p. 11) becoming less influential.
What can go wrong
The secret of safe immunotherapy is to go at exactly the right speed for the immune system of the

individual being treated. The doctor should look for feedback from the immune system – signs that show

how well it is coping with the steadily increasing dose of allergen – and use these to pace the

immunotherapy schedule.
Going too fast – getting ahead of the immune system’s ability to cope – is hazardous. A major allergic

reaction, called anaphylaxis (see p. 58), can occur, and this is the cause of deaths during

immunotherapy. However, as long as there is injectable adrenaline (see p. 150) and resuscitation

equipment available, even such an extreme crisis can be dealt with safely.
Serious reactions to immunotherapy usually occur:
•    during the initial build-up phase; maintenance therapy is much safer
•    during the pollen season, for those with pollen allergy
•    when a new vial of allergen extract is first being used, because of variations in concentration

(see p. 168-9).
Those most vulnerable to severe reactions are:
•    people with asthma, especially severe or unstable asthma
•    those who have experienced systemic allergic reactions in the past
•    anyone who appears to be extremely allergic, on the basis of skin tests
•    anyone taking beta-Mockers (see box on p. 150).
With care, these fatalities can be avoided. Patients who are given immunotherapy can ensure their own

safety by being well informed about the procedure (see p. 167).
The timing of immunotherapy
There are various different approaches to the timing of immurotherapy. The basic method (which has a

good safety record in the United States where it is very commonly used) starts with injections once a

week. After the maintenance dose has been reached, maintenance injections are given once every 2-4

weeks. The frequency of these may be increased during the pollen season, for people with pollen

allergies.
It is the regularity of the injection schedule that gradually creates, and then sustains, immune

tolerance, so the treatment is only of value to patients who can reliably keep their appointments.
When immunotherapy is successful, it can eventually be discontinued without any reappearance of the

allergic reaction. It usually takes 4-5 years of regular therapy, from the time of the first injection,

to get to this point. The benefits then persist for many years, perhaps indefinitely in some people,

even without any further injections.
Rush immunotherapy
Trying to speed up the process of immunotherapy greatly increases the risk of a severe reaction

(anaphylaxis). However, there are some situations where fast results are needed, and in such cases rush

immunotherapy, also called accelerated immunotherapy, may be used.
During the build-up stage of rush immunotherapy, injections are given every day, or even several times

a day. All the usual safety procedures (see below) are observed with particular care, to reduce the

chance of a severe reaction.
In semi-rush immunotherapy, the build-up injections are given twice a week, and the risks are lower

than with daily injections, but still higher than with weekly injections.
Minimising the risks
If you are lucky enough to be offered immunotherapy treatment under the National Health Service, you

should not feel concerned about accepting the offer. There is very little risk of a bad reaction

because safety procedures are now so stringent.
To minimise the risk of suffering a severe reaction, the doctor will ask you, at each visit, about any

reactions that occurred after your previous injection. Reactions might include redness, itching or

swelling around the injection site, or (more seriously) symptoms elsewhere on the body, such as nettle

rash (urticaria), itchy skin, sneezing, a runny nose, red or itchy eyes, tightness in the throat or

chest, coughing or wheezing. Always make a note of such symptoms, so that you don’t forget to mention

them at the next visit. This is crucially important, as such reactions can indicate that the immune

system is being hurried along too fast.
The doctor will also ask if you have an infection of any kind, as this can alter your reaction. You

should also tell the doctor about any new medicines being taken, as some, such as betablockers (see box

on p. 150), can make a bad reaction to the injection more likely to occur.
Asthmatics can expect the doctor to ask about current asthma symptoms, and to check their peak flow

both before and after an injection. If there are any symptoms, or if the peak flow is less than 70% of

the best-ever value, the injection won’t be given.
Severe reactions can sometimes begin several hours after the injection, so stay within reach of a phone

for about 24 hours. Among United States allergists (who don’t require their patients to wait after the

injection for more than 20-30 minutes) there are some who believe that everyone undergoing

immunotherapy should carry an adrenaline (epinephrine) auto-injector (see p. 150) on the day an

injection has been given, for use in the event of a severe reaction. Anyone who has suffered

anaphylaxis in response to an insect sting will probably have an adrenaline auto-injector anyway, and

this can certainly be used to treat anaphylaxis following immunotherapy. Note, however, that using the

adrenaline is just the first step in treating anaphylaxis (see p. 98) and you must then go back to your

allergist, or to the nearest hospital emergency department, without any delay.
It is sensible to avoid exercise for two hours after an injection. Be extra-cautious during the pollen

season if you are receiving immunotherapy for pollen allergies.
Immunotherapy for insect-sting allergy
`Our daughter has had two really bad reactions from being stung by a wasp. After the second one, the

doctor at the accident and emergency department told us that she nearly died. We got so anxious about

it that we worried every time we left the house in the summer, and it was even worse if she went out

without us. My wife got so upset about it that she wasn’t sleeping well. It was affecting the whole

family badly.
‘Then we heard about desensitisation treatment, and asked our GP, but he said he couldn’t do it.

According to him, they might be able to do it at the hospital, but it might not work, and it was risky

too. We accepted that at first, but then I started doing some research on the Internet, and discovered

that in America and Germany this treatment is absolutely standard – someone like our daughter would

automatically be given it. We felt very angry when we found this out, and went back to the doctor.

Eventually Ann was referred to the allergy department at a hospital, and now she is getting this

desensitisation treatment. I’m pleased about that, obviously, but I still think it shouldn’t have been

such a fight to get it.’
Immunotherapy provides highly effective protection for those with insect-sting allergy, and should be

given to anyone who has had a severe systemic reaction (see p. 60). Some United States allergists also

recommend it for adults who have had a cutaneous systemic reaction (see p. 60), on the basis that they

may well progress to a severe systemic reaction with the next sting.
Studies of people who have suffered severe systemic reactions, and are then treated with immunotherapy,

show that 97% have no systemic reaction to future insect stings. For the 3% who are not fully

protected, the severity of the reaction is much reduced and far less likely to be life-threatening. In

other words, this is an excellent treatment which can save lives.
Targeting the treatment
Choosing the right venom for immunotherapy can sometimes be difficult. Not everyone with insect-sting

allergy sees the insect that caused the reaction. Skin tests may not give the answer either, because

there are often positive reactions to several different venoms. Some of these may be false positives

(see box on p. 91) and it is impossible for the allergist to say which one(s) are actually relevant.

Most allergists will recommend immunotherapy for all of them, using a mixture of venom extracts.
Where the guilty insect was seen and identified, but other venoms also give positive skin tests, a more

difficult decision has to be made. Many allergists carry out immunotherapy for all the venoms that gave

a positive skin test, on a ‘better safe than sorry’ basis. Since there are cross-reactions between

venoms (see box on p. 113), there is some sense in this. Other allergists just give immunotherapy for

the insect that did the deed.
Will immunotherapy against one insect protect against a related insect? With two closely related

insects such as wasps and hornets, which have many allergens in common, it might do – but there is no

guarantee. The problem is that, as well as the shared allergens, each venom also has its own unique

ingredients. It’s impossible to say, with the kind of tests available at present, if an allergic

reaction was to shared allergens or unique ones. So immunotherapy against wasp venom may give

protection against hornet venom, but it will not necessarily do so – and vice versa.
In the case of bumblebee allergy (seen almost exclusively in those, such as horticulturalists, whose

work involves handling bumblebees) a more definite answer can be given – honeybee immunotherapy does

not work. Immunotherapy with bumblebee venom does work, fortunately. The bumblebee extract has to be

obtained from specialist sources.
Injections are given weekly during the build-up phase, unless protection is needed urgently, as with

work-related sting allergy, in which case rush immunotherapy may be used. Once the maximum dose has

been reached, a maintenance injection is needed every four weeks. After a year, this maintenance dose

can be given every 6-8 weeks.
After 3-5 years of immunotherapy, skin tests with insect venoms are usually tried again. If the results

are negative, the immunotherapy will stop. Research now shows that, even if skin tests are still

positive when immunotherapy ends, there’s an 8090% chance that no systemic reaction will occur to

future stings. Some people are not reassured by this, and prefer to continue with immunotherapy for

their own peace of mind. Indeed, research shows that a near-fatal systemic reaction has a long-lasting

psychological impact, and that many people continue to feel anxious despite completing immunotherapy

and reacting negatively to skin tests.
At one time, challenge stings with live insects were given to check whether immunotherapy had actually

worked. Few doctors do this now, but your allergist may be prepared to do a challenge test if you ask.

Adrenaline and resuscitation equipment would be available if a challenge test were used, so any severe

reaction could be dealt with promptly and effectively. The fact that the psychological consequences of

insect-sting allergy are so persistent suggests that challenge tests with live insects may have a

particular value, in demonstrating that immunotherapy has worked. Challenge tests are also helpful for

those who work with stinging insects, such as honeybees and bumblebees, and who need to be sure that

they can go back to work safely.
Immunotherapy for food allergy?
Attempts to use standard immunotherapy for food allergy have been made repeatedly, but without success.

The process of giving the injections is nerve-racking because of the constant risk of a severe

reaction. The risks prevent the dose of allergen being increased very much, so the beneficial effects

are small. While there may be some reduction insensitivity, it is not enough – or not reliable enough –

to be of any practical value.
What doctors are aiming for here, incidentally, is simply to protect against the effects of

accidentally eating a tiny amount of the food – no one is expecting that someone with peanut allergy

will be able to eat peanut butter sandwiches as a result.
Some of the new developments in immunotherapy may be useful for food allergy, as described in the next

section.
The future of immunotherapy
Many different research teams are working on ways of improving immunotherapy – making it more

effective, safer to give, and less time-consuming.
One approach involves altering the allergen, so that it only interacts with those parts of the immune

system whose job is to control allergic reactions (and therefore bring about tolerance). The changes

made to the allergen are designed to make it ‘invisible’ to the parts of the immune system that

actually attack the allergen. The idea is to inject something that can’t cause a bad reaction, and is

therefore 100% safe.
The modified allergens are called allergoids. Another term often used is peptide immunotherapy – this

describes a technique in which the allergens are chopped up into small pieces to make them safe

(allergens are proteins, and a fragment of a protein is called a peptide).
Already, researchers in Germany have made an allergoid from birch pollen that can reduce hayfever

symptoms with a series of just seven injections given before the pollen season.
Meanwhile, a research team in London is working on peptides made from cat allergen, with encouraging

results so far. In a group of asthmatics who were allergic to cats, a series of 4-10 injections, over a

period of 2-8 weeks, produced benefits in about half those treated. The researchers believe that they

can improve on this and help the majority of people with cat allergy, at least enough to survive

temporary exposure to cat allergen (when visiting cat-owning friends, for example). They hope to refine

the treatment sufficiently to enable some cat-allergic people to keep their pet, rather than finding it

a new home. This is a relatively safe treatment that might be given by a GP, rather than only by

specialists. The research team hopes the treatment will be available by about 2009.
Could this kind of technique work for food allergy? Doctors believe that it can, and a great deal of

research work is being done, in both Britain and the United States. A major focus of this effort is

peanut allergy, since this puts so many young lives at risk. Even if the research is successful, It

will be several years before such treatments become available.
Researchers are also working hard to produce standardised allergen extracts – in other words, allergen

extracts that always contain a standard amount of the allergen. The aim is not only to reduce the

number of treatment failures (which can occur if the extract does not contain enough allergen) but also

to avoid mishaps when a new vial of allergen extract is used (differences in strength, between one vial

and another, are sometimes a cause of anaphylactic reactions).
Standardisation is difficult, because the starting materials –skin particles from horses, for example,

or dust-mite droppings –are natural materials and therefore variable. Some samples contain far more of

a particular allergenic ingredient than others.
One way around this problem is to develop accurate methods of measuring the amount of allergen in the

extract. Another approach is to abandon the whole business of making extracts, and produce allergens

artificially, in a laboratory. This is done by inserting the gene for the allergen – the gene for the

Der p1 allergen of house-dust mite, for example – into bacteria. These bacteria then act as production

units, manufacturing large amounts of the allergen every day. With this high-tech approach, the exact

content of the allergen preparations can be controlled.
These high-tech allergen preparations are extremely pure, and therefore very effective – as long as the

person receiving immunotherapy really is sensitised to the particular allergen that is included.

Unfortunately, most natural allergenic materials contain two, three or even more separate allergens. In

house-dust mite droppings, for example, while Der p1 is the allergen that affects most people, there is

also an allergen called Der p2, and a few people are more sensitive to this than to Der pl.
Artificially produced allergen preparations usually include the main allergen only. For the minority of

people who are more severely allergic to one of the other allergens, this extract will not work.

Eventually this problem will no doubt be circumvented by means of more precise skin testing before

immunotherapy begins – skin tests with individual allergens, rather than with allergen extract

containing a mix of allergens.
A third approach is to change from injections to oral immunotherapy – giving the allergen extracts by

mouth. The best results are obtained when the allergen is held under the tongue for a while and then

swallowed. This is known as Sub-lingual immunotherapy or SLIT, and has become very popular in Italy and

France, where it is a common treatment for hayfever. A recent pilot trial among GPs in Britain suggests

that it may be useful, but is not a miracle cure. Overall, the group treated with SLIT had fewer

symptoms during the pollen season, but antihistamines were still needed. There is some evidence from

Italy that SLIT might reduce the likelihood of children with hayfever going on to develop asthma, and

reduce the chance of new sensitivities.
Side effects are unusual with this treatment, and those that do occur are mostly mild – itching in the

mouth, for example. The treatment is safe enough for routine use in children.
Might oral immunotherapy work for food allergy? Other Italian studies suggest that it could. The

objective of these studies is to reduce the risk to children with cow’s-milk allergy from accidental

encounters with ‘hidden milk’ in prepared food or drink. The immunotherapy treatment begins with

miniscule amounts of milk – the doctors start with a single drop diluted in water, each day for a week

– and increase the dose extremely slowly. Antihistamines are given to minimise the risk of a reaction.
The whole process requires enormous patience, but after seven months, the majority of the children

involved can tolerate some milk – between three tablespoonfuls and a small cupful each day.
This is a very encouraging study that should be repeated by doctors in Britain. Because of the risks of

anaphylaxis – which can, of course, be fatal – it does require full medical supervision, and you should

not attempt it at home. Whether this method would work for allergens other than milk is something that

nobody has yet investigated.
A great many other approaches to immunotherapy are currently being tried for food allergy. Many of the

new techniques are highly experimental, and some show great promise, but it will be many years before

they are in use.
One innovation that is closer to being in general use in the United States involves giving the anti-IgE

drug omalizumab (see p. 149) alongside immunotherapy injections. The drug maximises the benefits from

the immunotherapy, and may make the build-up stage (see p. 165) safer, by lowering the risk of

anaphylaxis. For British allergy sufferers, who cannot yet get omalizumab, and whose chances of getting

immunotherapy are vanishingly small, it may seem unkind even to mention such treatments, but we can

only hope that things will improve here in the near future. You might take some comfort from the

thought that, by the time immunotherapy is available again in Britain, there will be a whole host of

highly effective new techniques available for doctors to try.
All the methods described above are forms of specific immunotherapy – they treat an allergy to dust

mites or to grass pollen or some other specific allergen.
A far more radical and ambitious approach to immunotherapy is now the aim of some medical researchers:

blocking the tendency to allergies as a whole.The underlying idea here is to reverse the basic shift in

the immune response, from Th1 cells to Th2 cells. It is this shift to Th2 cells which produces the

allergic tendency (see pp. 11 –13).
Some interesting findings have already been made in this area, including the surprising discovery that

the balance of Th1 cells and Th2 cells can be adjusted even in people with longstanding allergies.

Inspired by discoveries about hygiene and allergy (see p. 21), British researchers have made a vaccine

containing inactivated cells of a harmless bacterium found in the soil, Mycobacterium vaccae. This is

given as a single injection just under the surface of the skin. It has been used for adult patients

with asthma, and for children with severe atopic eczema, with some improvement in both groups. If the

treatment proves as useful as the preliminary studies suggest, this could be a common treatment in a

few years’ time.

Atopic eczema
A Greek word meaning ‘to boil over’ or ‘to erupt’ is the source of the medical term ‘eczema’. It refers, of course, to the way in which the skin erupts into a rash, but it could equally well describe the eruption of controversy around this disease. No other allergic disease is quite such a cauldron of dissent - indeed, even the question of whether it is an allergic disease remains unresolved. These controversies directly affect the treatment of atopic eczema, so it is useful to understand them if you or your child have eczema.
The disagreement begins with the question of what causes atopic eczema.
Let’s start with the one point that everyone agrees on: dry skin plays a fundamental role. Those with atopic eczema have dry skin, not just in the eczematous areas, but in other parts as well, sometimes all over the body. The skin cells are less efficient than normal skin cells at retaining water.
Everyone would also agree that there is inflammation of the skin – a reaction that is produced by the immune system. But when it comes to the question of what starts off the inflammation there are huge differences of opinion among specialists treating atopic eczema – these specialists include dermatologists, allergists and paediatricians.
Since people with atopic eczema are atopic (allergy-prone), and most have
huge amounts of the allergy antibody, IgE, going round in their blood, it might
seem plausible that an allergic reaction to some external item kicks off the
inflammation. And when skin-prick tests (see p. 91) to common allergens such
as house-dust mite are tried, there are usually a large number of positive results.
But many of these turn out to be false positives – when tested more directly,
the allergen concerned does not actually play a part in causing the skin symptoms.
This has led some specialists working with eczema, mainly dermatologists, to
What the words mean
Eczema is not a disease in itself. The word refers to a certain type of reddish rash — a rash which can be caused in a variety of ways. The type of eczema that affects people of an allergic disposition (atopics), is called either atopic eczema or atopic dermatitis.
The word dermatitis just means inflammation of the skin. Most doctors consider it to be synonymous with eczema, but some give it a slightly broader meaning.
believe that allergic reactions play little part in either initiating or perpetuating atopic eczema. In their view, the basic cause of atopic eczema is dry skin and a generally overwrought immune system, not specific allergic reactions.
To some of these doctors, positive skin-prick tests are all false positives in atopic eczema – that is, irrelevant to the disease process. A positive skin-test result, in their opinion, simply indicates that the skin of atopic eczema sufferers is in a highly sensitive state, not that the allergen concerned plays any causative role.
Allergists tend to take a different view of this, as you might expect. And recent research shows that they are correct – allergens often do play a significant part in provoking atopic eczema.
Research using direct challenge tests (see p. 90) has identified some of the things that could provoke such sensitivity reactions:
• house-dust mites, pollen or moulds
• cats, dogs, rabbits and other furry pets
• cow’s milk or other food – a prime suspect in babies and young children (see p. 68). The response to food is usually delayed, occurring some hours after the item is consumed.
With mite, pollen and pet allergens, the eczema symptoms can be provoked either by allergens falling on the skin, or by direct contact (e.g. mite allergens in the bed, skin contact with pets, or lying on grass for those with grass-pollen allergy).
The rash tends to occur on skin not covered by clothes, as you would expect. But it can sometimes occur only on particular exposed areas – usually the most sensitive areas of skin. For example, there are people who react to house-dust mite but have eczema on the eyelids only.
Additionally, experiments show that even when an airborne allergen is only inhaled it can sometimes provoke eczema symptoms. The allergen probably reaches the skin in the bloodstream. (Alternatively, it might provoke an immune reaction in the airways which generates chemical messages of the kind that promote inflammation – these then reach the skin in the blood.) This means that the skin reaction could occur anywhere on the body, not just on exposed skin.
In the case of food, the molecules of food that cause the trouble are probably being absorbed from the stomach without being completely broken down. They then reach the skin via the blood to provoke a reaction there. (Or, again, it could be an inflammatory messenger chemical travelling from the gut to the skin in the blood.)
When food gets directly onto the skin – which it frequently does with small children, of course – it can provoke a reaction that way too. This may be a slow eczema-causing reaction, or a much faster reaction known as contact urticaria (see pp. 50-51). Reacting to food with contact urticaria is quite common in children with atopic eczema – but the same food doesn’t necessarily provoke atopic eczema when it is eaten. (However, eating these foods can sometimes trigger anaphylaxis – see pp. 58-9. They should therefore be treated with great caution.)
At the same time as all this research – which shows for sure that allergens play a part in atopic eczema – others have been asking what actually happens when skin reacts to an allergen. Their studies have turned the accepted understanding of allergies upside-down. They show that when something like egg or pollen provokes atopic eczema, what is occurring isn’t necessarily an allergic reaction of the usual sort, with IgE and mast cells (see
box on p.12). Instead, other immune cells are causing the trouble. Sometimes IgE is involved, but without mast cells. Sometimes neither is involved. These revolutionary discoveries are described in more detail on pp. 18-19. One interesting realisation from this research is that in different eczema sufferers, different immune reactions may be producing the rash – even if they are reacting to the same allergen! This helps to explain why the results of skin tests are so inconsistent and puzzling.
The wandering rash
For a baby with atopic eczema, the face, and especially the cheeks, are commonly affected, but there may be a rash all over the legs, the backs of the arms, and the back. As the months go by, the rash settles on the lower legs, and spreads to the fold of the elbow, and then the fold at the back of the knees — by about three years of age, this flexure eczema is the main problem for most children.
In adults, eczema is often found in quite restricted areas, such as the hands, scalp, lips, eyelids or chest. It may be located around the nipples — a sensitive spot where rubbing by clothing is enough to initiate a rash.
Atopic eczema is always in a process of change, and different parts of the body may display different stages of the rash:
• The rash is red and usually dry at first, and there may be not a great deal to see. In this early stage the visible signs may be minimal, while the itchiness can be colossal. Sometimes there is oozing of clear fluid.
• Occasionally the first phase is more marked, with dense patches of small red bumps or tiny blisters. On the hands, these may merge to form larger blisters.
• Infections tend to change the appearance of the rash (see p. 44).
• With time the skin becomes thicker, paler and scaly. It may form leathery patches (called lichenification), especially if there is habitual scratching or rubbing. This is chronic eczema.
• When the eczema clears, there may be an area of skin that is lighter in colour, or darker, than the surrounding skin.
The next step
Whatever causes atopic eczema, it provokes the most horrendous itching, as every eczema sufferer knows. The itch cries out to be scratched, and scratching is the major cause of the visible rash. If left untouched, the skin does not erupt into eczema, although it may well turn red, and there are still distinct changes in the skin that can be seen with a microscope.
Once eczema has erupted, the skin is no longer an intact protective layer that neatly separates ‘in-here’ from ‘out-there’. The skin becomes more permeable and loses its own natural moisture far more readily, so the dryness gets worse. At the same time allergens and irritants penetrate far more easily, causing yet more inflammation.
Something else compounds the damage: once atopic eczema is established, the immune system starts making IgE antibodies to the body’s own proteins, especially those found in skin cells. This helps explain why atopic eczema can become so severe and so entrenched.
Infections — another vicious circle
When eczema erupts and the skin barrier is breached, infections often become a problem. A regular source of trouble is the bacterium Staphylococcus aureus, a cause of the infection impetigo. This microbe invades eczematous skin far more readily than healthy skin, causing a prolific ooze with golden-yellow crusting.
Staphylococcus aureus produces a toxin known as a ’super-antigen’ which revs up the immune system to even more furious effort. This effort does not, unfortunately, oust the bacteria, but it does make the skin inflammation even worse. To add to their woes, many who are afflicted with atopic eczema start making IgE antibodies against Staphylococcus aureus toxins.
Infection with fungi (yeasts and moulds) is also a problem in atopic eczema (see p. 49), and there may be sensitivity reactions to these fungi.
The herpes virus, responsible for causing cold sores, can also invade eczematous skin, though this is much rarer. It worsens the eczema and produces fever and general weakness. There may also be flocks of small red bumps, each with a tiny dimple or blister at the centre. Any symptoms of this kind indicate that the patient needs urgent treatment.
Irritants and stress
People with atopic eczema are far more susceptible to everyday irritants such as wool and rough synthetic fabrics, soap, and traces of detergent left behind in clothes. Chlorinated water, either in swimming pools or from the tap, can also aggravate the skin, and even ‘hard’ water (found in areas with chalk or limestone bedrock) may be a factor.
Some air pollutants may play a part in atopic eczema. Researchers in Germany have found that children living close to busy trunk roads, or in homes with a gas cooker and no extraction hood (see pp. 128-9), were more likely to develop eczema. Formaldehyde fumes, often found in modern houses (see p. 129), are sometimes a factor when eczema affects the face and hands.
